Objective1.To evaluate the left ventricular regional and global function ofpatients with diabetes and prediabetes by strain rate imaging and Teiindex for the sake of providing evidences for diabetic cardiomyopathy’searly diagnosis and therapy.2.Strain rate imaging, Tei index, conventional left ventricularsystolic and diastolic function parameters were analyzed by linearcorrelation and evaluated their clinical application.Methods35cases of diabetes,30cases of prediabetes and35cases of controlsubjects were included in this study.Firstly, the left ventricularfunction indicators(EF,FS,SV,E/A,) were measured by conventionalechocardiography.Secondly, mitral annular velocity(Ea/Aa)and Tei indexwere measured with TDI.Thirdly, the peaks of strain rate curve atsystole(SRs),early diastole(SRe), late diastole(SRa) and SRe/SRa at thebase and mid segments were measured by strain rate imaging.Lastly,strain rate imaging, Tei index, conventional left ventricularsystolic and diastolic function parameters were analyzed by linearcorrelation.Results1.In control group, left ventricle longitudinal strain rate showed thehomogeneity in different segments and there were no significantdifferences between different segments (P>0.05).2.The SRs,SRe,SRe/SRa measured in prediabetic patients group weresignificantly lower than those in control group(P<0.01),but thedifference in (EF,FS,SV,E/A,Ea/Aa) had no statistical significance(P>0.05). Tei index Was longer in prediabetic patients group than in control group(P<0.01).3.The SRs,SRe,SRe/SRa, E/A, Ea/Aa measured in diabetic patients groupwere significantly lower than those in control group and prediabeticpatients group(P<0.01), the difference in (EF,FS,SV) had no statisticalsignificance(P>0.05). Tei index Was longer in diabetic patients groupthan in control group and prediabetic patients group(P<0.01).4. Tei index was negative correlated with EF,FS, E/A, Ea/Aa,thecoefficient was-0.810,-0.755,-0.358,-0.316respectively(P<0.001).5.①Mean SRs of the base segments was positive correlated with EF,FS,the coefficient was0.471,0.509respectively(P<0.001).②Mean SRe ofthe base segments was positive correlated with E/A、Ea/Aa, the coefficientwas0.593,0.555respectively(P<0.001).③Mean SRe/SRa of the basesegments was positive correlated with E/A、Ea/Aa, the coefficient was0.794,0.768respectively(P<0.001).6. Tei index was negative correlated with Mean SRs,SRe,SRe/SRa of thebase segments,the coefficient was-0.602,-0.574,-0.374respectively(P<0.001).Conclusions1. In normal group, left ventricle longitudinal strain rate showed thehomogeneity in different segments.2. Left ventricular regional systolic and diastolic function of patientswith prediabetes were damaged.3. The technology of strain rate imaging and Tei index are more sensitivethan conventional left ventricular systolic and diastolic functionparameters and can diagnose the myocardial disease of diabetes earlier.4.The correlation coefficient among strain rate imaging、Tei index andconventional left ventricular function parameters was high and they couldbe united to comprehensively evaluate left ventricular regional andglobal function.
