Background: Idiopathic nephrotic syndrome is the most common type in nephroticsyndrome. All pathological types can cause the damage of glomerular endothelialcells. The common complications are thrombosis and embolism, and thrombusformation is a major factor for glomerular sclerosis. So anticoagulation is one of theroutine preventive treatment of idiopathic nephrotic syndrome; Thrombomodulin as acondition for monitoring, diagnosising and an indicator to measure standards of carein diseases associated with endothelial cell injury,also an important marker ofvascular endothelial cell injury. The level of detection are more and more attentioned.Numerous clinical studies results show that thrombomodulin has become an importanttool used to monitor the disease and observe the condition in the clinical, However,the clinical applications of the thrombomodulin level monitoring in renal disease isnot very widely; Fibrinogen also named antihemophilic factⅠ, is synthesized in theliver, an acute-phase plasma glycoprotein. Fibrinogen can stimulate endothelial cellsand smooth muscle cell synthesis and secretion of endothelin and inhibition of nitricoxide synthase and nitric oxide, causes vascular injury in the role of procoagulant; thecorrelation in serum thrombomodulin and fibrinogen has not been reported indifferent pathological types of idiopathic nephrotic syndrome, This article intends tostudy the expression levels of different pathological types of INS patients sTMwhether differences exist, thrombomodulin and fibrinogen expression level whether there is a correlation, and complicated by thrombosis and embolism seen as abarometer for the idiopathic nephrotic syndrome to provide theoretical basis.Objective: To discuss the correlation and the expression of serum thrombomodulinand fibrinogen in idiopathic nephrotic syndrome,analyze the realationship betweenthem and renal pathology type in patients with INS.Materials and methods:60patients were diagnosed by renal biopsy and30healthycontrols serums were collected and examined by enzyme linked immunosorbent assay(ELISA)and plasma were examined by thrombin turbidimetry.Results: The serums of patients with INS significantly higher than healthy controls(P<0.05),also MN controls lower than the other controls of INS (P<0.05). Thecorrelation analysis of the serums of patients with MN controls and MsPGN controlsdetermined negative correlation between serum thrombomodulin (sTM)andfibrinogen(FG)(r=-0.441、r=-0.595,P<0.05).Conclutions: The levels of sTM not only can be sensitive indicators with the damageof glomerular endothelial cells, but also provide some guidance for clinicalobservation of Pathology type of MsGPN and MN complicated by thrombosis andembolism in INS.
