【Objective】Using Real-time quantitative PCR investigate the expression of miR-21in diffuselarge B cell lymphoma and normal lymph tissues, to analysis and the expression levelDLBCL patients with clinical features and prognosis of the relationship, to clarify themiR-21expression in DLBCL occurrence and development of significance, to explore therelationship of P53, Bcl-2protein with miR-21, provided the basis for the DLBCLmolecular targeted therapy.【Methods】1.Cases collection:Collected and filtered50cases of DLBCL organizationspecimens with complete clinical information and living conditions,and12cases of normallymph node.2.Determine the sources of B-cell tumor cells:The immunohistochemistry EnVisionmethod detected expressionof protein with LCA,CD45RO and CD3,CD20.3.All of case classification:Hans’ typing methods marking of CD10, Bcl-6andMUM-1protein,two cases were classified into germinal center B-cell (GCB type) andnon-germinal center B cell type (non-GCB is type) according to their expression.4.Used Real-time quantitative PCR detect the expressions of miR-21and statisticalcorrelation analysis associated clinical factors and clinical outcome.5.Used immunohistochemical to detect the expressions of Bcl-2and P53protein,correlation analysis of the P53protein, Bcl-2and miR-21expression.【Results】1.In GCB subtypes and non-GCB is subtype of this group of DLBCL cases (50) were 19cases (38.0%),31cases (62.0%);the subtypes GCB is the survival rate is higher thannon-GCB is subtype (p=0.039).2.The miR-21high expression and miR-21low expression of this group of DLBCLcases were34cases (68.0%),26cases (32.0%);high expression of miR-21DLBCL patientsthe survival rate was lower than that of miR-21low expression in patients (P=0.038).3.The P53positive expression and negative expression of this group of DLBCL caseswere21cases (42.0%),29cases (58.0%);and P53expression in the patient’s survival ratewas significantly lower (p=0.017).4. The expression levels of miR-21was negatively correlated Bcl-2protein level (p=0.02) and positively correlated P53(p=0.022).5.The single-factor Cox analysis and multivariate Cox analysis was in the50cases ofDLBCL patients (gender,location,diameter,lymph node metastasis,stage) and theexpression of miR-21,Bcl-2,P53.The univariate results found the expression of miR-21(p=0.046),immune subtype (p=0.046),P53(p=0.022) and clinical stage (p=0.022) withthe prognosis was statistically significant,other factors were not statistically significance.The multi-factor Cox analysis showed that the expression of miR-21(p=0.002),staging (p=0.000),p53(p=0.007) and immune subtype (p=0.000) were statistically significant,other factors were not statistically significant.【Conclusion】1.The prognosis of GCB subtype of DLBCLs is better than that of non-GCB subtype.2. Using Real-time quantitative PCR,the expression of miR-21in DLBCLwas high,the expressionof miR-21in non-GCB is higher than in GCB, the patient survival rate ofhigh-expression miR-21was significantly higher than the low-expression miR-21ThemiR-21was carcinogenic effects in DLBCL.3.The detection of P53,Bcl-2protein expression contribute to the prognosis ofDLBCL patients, to further improve the clinical stage, and to provide a reasonable basisfor the patients.4. The target proteins of miR-21may be Bcl-2, p53, to provide a theoretical forbasistargeted therapy.5. The clinical stages, miR-21, P53, immune subtypes of DLBCL is can be used as aprognostic indicator.
