ObjectiveTo investigate the frequency of systemic lupus erythematosus(SLE) complicatedwith immune-related hemocytopenia(IRH), and to explore the possible mechanismand its clinical significance.MethodsNine patients with SLE and five healthy controls were enrolled. Both StandardCoombs test and Bone marrow mononuclear cells (BMMNCs) Coombs test wereperformed to detect autoantibody on red blood cells or BMMNCs.The correlationbetween the type of auto-antibodies on BMMNC and clinical manifestation wasanalyzed.ResultsIn these9SLE complicated with IRH,2had simple anemia,4anemia andthrombocytopenia,2anemia and neutropenia and1pancytopenia. Positive results ofBMMNC-Coombs test were seen in2patients (2/9), which was significantly higherthan that in healthy controls(0/5). Positive results of standard Coombs test were seenin2patients too,one of which was of positive result in BMMNC-Coombs test.In these2cases with autoantibody on BMMNCs,1had simple IgG antibody and2had IgG+C3. Both patients experienced severe anemia. Clinical manifestation variedfrom the different types of autoantibodies on BMMNCs. In both cases withautoantibody on peripheral blood cells, the autoantibodies were IgG+C3.There was a significant correlation between the auto-antibodies on BMMNC andserum anti-SSA.ConclusionsAutoantibody may exist on BMMNC in patients with SLE.In SLE patients with IgG autoantibody on BMMNCs the main clinicalmanifestations was severe anemia.There was a significant correlation between the auto-antibodies on BMMNC and serum anti-SSA.
