Objective: To compare the clinical effects and implications oftransforaminal lumbar interbody fusion(TLIF) and posterior lumbar interbodyfusion (PLIF)on moderate and severe lumbar spondylolisthesis by aretrospective study.Methods:From Jan.2004to Jan.2008,48patients with moderate and severelumbar spondylolisthesis were treated with PLIF or TLIF in our hospital,25cases undertook PLIF,23cases undertook TLIF. Examined the difference ofsurgery time, blood loss, bed time, Nakai score,fusion time(according to theSuk method), postoperative complication, disc height and foraminal height ofpreoperative, postoperative and the end of follow-up between groups.Results:All the patients recovered uneventfully with the good healing of theincision and were followed up.The follow-up period was1-4years, mean3years. All patients achieved spinal fusion. There was no significant differenceon bed time, Nakai score, fusion time, disc height and foraminal height at thesame time between groups(P>0.05).There was statistically significantdifference by comparing surgery time, blood loss and postoperativecomplications(P<0.05).In two groups disc height and foraminal height ofpostoperative and the end of follow-up were significantly improved comparedto preoperative(P<0.05). Of the48patients, intraoperative dural lacerationoccured in2cases(0in TLIF group,2in PLIF group), by using noninvasivesilk suture, or local repair with its own muscle tissue or artificial hard present,there was no postoperative cerebrospinal fluid (CSF).Conclusions: Interbody fusion with either a PLIF or a TLIF provides goodoutcomes in the treatment of moderate or severe lumber spondylolisthesis;compared to the PLIF, the TLIF procedure is more simple, less blood loss andcomplication.
