Objective:Systemic lupus erythematosus (SLE) is a diffuse connectivetissue disease,that multisystem multiorgan involvement. The pathogenesis iscomplex, involving immunological, genetic, hormonal, environmentalprecipitating factors and various immunological abnormalities. Regulatory Tcells (Tregs) plays an important role in immune responses and immunetolerance,Many studies suggest that the number decrease or functionalabnormalities of CD4+CD25+regulatory T cells may lead to variousautoimmune diseases. The immune regulation, inhibition is mainly throughthree ways, that is direct intercellular contacts, secretion Inhibitory cytokines,the expression of transcription factor FOXP3. FOXP3is a newly discoveredtranscription factor, specifically express on the CD4+CD25+Treg cells, tomaintain the development of CD4+CD25+Treg cells in thymus, peripheralexpression and function. Studies have found that the number of CD4+CD25+Treg cells with FOXP3gene mutation or knockout mice decreased, can inducesevere autoimmune disease. FOXP3gene mutations can be inducedautoimmune disease, infusion of FOXP3+Tregs are reversible. BecauseFOXP3is a new kind of transcription factor, now there are differentresults to the study.This study aims to test the level of FOXP3mRNA in peripheral bloodmononuclear cells of systemic lupus erythematosus patients, to know thechanges of its expression in patients with SLE, explore the role of FOXP3playin SLE morbidity, and discuss its significance, provide new ideas for thetreatments of SLE.Methods:1.65cases were chosen and they were divided into three groups:(1) Early systemic lupus erythematosus group:22cases of early SLE patients;(2)remission systemic lupus erythematosus group:25cases SLE patients inremission;(3) healthy control group:18cases is from healthy volunteers2. In this study, the level of FOXP3mRNA in peripheral blood mononuclearcells of the three groups were detected by RealTimePCR, to know thechanges of its expression.3. Records the clinical and laboratory data before and after treatment, andcalculate the SLEDAI activity score, compare the expression of FOXP3mRNA in peripheral blood mononuclear cells of every group. and with thelaboratory data for the correlation analysis.4. Statistics work was done with SPSS13.0statistical software.Results:1. The comparison of the expression level of FOXP3mRNA in peripheralblood mononuclear cells between the three groups: the expression level ofFOXP3mRNA in early systemic lupus erythematosus patients0.09(0.08)were higher than in remission systemic lupus erythematosus group0.07(0.08), P>0.05,and were higher than healthy control group0.07(0.11),P>0.05. The difference expression level of FOXP3mRNA betweenremission systemic lupus erythematosus group0.07(0.08) and healthycontrol group0.07(0.11) was not statistically significant P>0.05.2. Correlation cofficients for the expression level of FOXP3mRNA andother index in early systemic lupus erythematosus group: the expressionlevel of FOXP3mRNA had significant negative relationships withsystemic lupus erythematosus disease activity index (SLEDAI), r=-0.442,P<0.05; and didn't have obvious relationships with the alexineC3alexineC4, Urine protein quantitative of24hour, ESR.3.22cases in early systemic lupus erythematosus patients were divided intothree groupsb by SLEDAI:5cases in no activities group,6cases in mildactivities group,11cases in moderate and severe activities group. Thecomparison of the expression level of FOXP3mRNA between the threegroup: the moderate and severe activities group were significant lower than no activities group, P<0.05. compared with mild activities group,remission systemic lupus erythematosus group, healthy control groupthere were no significant difference. P>0.05. The comparison of noactivities group and mild activities group, there were no significantdifference. P>0.054. According to the24-hour urinary protein quantitative whether more than0.5g, the early systemic lupus erythematosus group is divided into lupusnephritis group10cases and non lupus nephritis group12cases: Thedifference of the expression level of FOXP3mRNA between lupusnephritis and non lupus nephritis was not statistically significant, P>0.05.5. The expression level of FOXP3mRNA in remission systemic lupuserythematosus group didn't have obvious relationships with the totalamount of cyclophosphamide, P>0.05.Conclusions:1. The comparison of the expression level of FOXP3mRNA between earlysystemic lupus erythematosus, remission systemic lupus erythematosusgroup and healthy control group, there were no significant difference.P>0.05, indicated that FOXP3didn't participate the morbidity process ofsystemic lupus erythematosus.2. The expression level of FOXP3mRNA had significant negativerelationships with SLEDAI, and the moderate and severe activities groupwere significant lower than no activities group, indicated that FOXP3isrelated with activities of systemic lupus erythematosus, and may becomeobserved one index of SLE activities.3. The difference of the expression level of FOXP3mRNA between lupusnephritis group and non lupus nephritis group was not statisticallysignificant, indicated that FOXP3is not related with renal involvement ofsystemic lupus erythematosus... |
