Objective:To investigate the correlation of plasma copeptin level and severity of myocardial ischemia injury and the impairment of coronary artery; to observe the relationship between copeptin and cardiac troponin I(cTnI) in patients with AMI and the diagnostic value of copeptin in early ACS.Methods:We selected 164 patients who had acute coronary syndrome (ACS) with onset<24h in our study. Coronary angiography (CAG) was performed in the patients and 30 normal coronary angiography (CAG) was in control group. The patients with ACS were divided into two groups, including unstable angina pectoris group and acute myocardial infraction group according different clinic type of acute coronary syndrome; the severity of pathological changes of the coronary artery was assessed by the number of diseased coronary branches and Gensini scores. The plasma copeptin was measured by enzyme-linked immunosorbent assay (ELISA) and the plasma cTnI was measured by immunofluorometric assay technique.Results:(1) The level of plasma copeptin was obviously higher in ACS group than that in control group (P<0.05). The level of plasma copeptin was higher in AMI group than that in UAP group (P<0.05). (2) The level of plasma copeptin in left main coronary lesion group was the highest; the level of plasma copeptin in two-vessel and three-vessel lesion groups was higher than that in one-vessel group (both P<0.05); the level of plasma copeptin in midrange and serious narrow groups was higher than that in gently narrow group (both P<0.05). (3) The level of plasma copeptin in the high risk ACS group was higher than that in intermedia-low risk groups; along with the copeptin grade higher, the risk of STEMI incidence was higher. (4) The level of plasma copeptin was negatively associated with the level of plasma cTnl in patients with AMI (r=-0.595, P<0.05).Conclusion:(1) The level of plasma copeptin has relationship with the degree of myocardial ischemia damage and the severity of coronary atery lesion. The plasma copeptin level provides a better risk stratification and helps us to judge the prognosis in patients with ACS and makes treatment protocols. (2)Copeptin increases earlier than cTnI in early AMI and there is negative correlation between copeptin and cTnI. So copeptin can be used as a diagnostic parameter in early ACS.
