Objective To explore the changes of ischemia modified albumin(IMA),soluble intercellular adhesion molecule-1(sICAM-1) and hypersensitive c-reactive protein(hs-CRP) levels in coronary heart disease(CHD) patients and their clinical significance respectively.Methods To detect the levels of IMA,sICAM-1 and hs-CRP in patients with acute myocardial infarction (AMI),unstable angina pectoris (UAP) and stable angina pectoris(SAP) in 20 cases respectively and 20 healthy subjects served as controls.Results The serum levels of IMA, sICAM-1 and hs-CRP in AMI patients were significantly higher than that of UAP, SAP and the controls(F=76.52,29.75.12.90, q=6.08-59.02,P<0.01), the levels of IMA, sICAM-1 and hs-CRP were significantly higher in UAP than that of SAP and the controls(q=4.30-181.90, P<0.01), and it was significant that the difference of levels of IMA, sICAM-1 and hs-CRP between SAP and the controls(q=4.97,8.58,2.82; P<0.01). The levels of IMA, sICAM-1 and hs-CRP showed a positive correlation between different type CHD (Between the IMA and sICAM-1 r=0.537, between the IMA and hs-CRP r=0.699,also between the sICAM-1 and hs-CRP r=0.543, P<0.01). The areas of levels of IMA, sICAM-1 and hs-CRP under the ROC curve(AUCROC)were all greater than 0.82 with the sensitivity and specificity of 0.90,0.80,0.75,0.85 and 0.80,0.77 in AMI. And the areas of levels of IMA, sICAM-1 and hs-CRP under the ROC curve(AUCROC) were all greater than 0.74.with the sensitivity and specificity of 0.85,0.95,0.60, 0.75 and 0.70,0.90 in SAP screening. False positive rate were 0.05,0.25 and 0.10 in SAP screening.Conclusion The changes of IMA, sICAM-1 and hs-CRP have a great value in the diagnosis of AMI and CHD screening. They all can significantly enhance the diagnostic accuracy and sensitivity of CHD. The rising degree of IMA, sICAM-1 and hs-CRP may be used as one of the indexes for estimating the severity of the disease and prognosis.
