PurposeTo classify the segmental bronchial patterns of the right lung segmental bronchus by the application of post-processing of 16-slice CT.Methods232 patients with routine thorax scans were enrolled. The segmental bronchi were demonstrated in terms of bronchial multiplanar reformation (MPR) technique,bronchial tree and virtual bronchoscopy (CTVE), Analysis of different methods of display and Calculate the probability of the form. The identification of segmental bronchi depends on bronchial lumen diameter and branching direction of 3D bronchial tree. The classification of segmental bronchial branching patterns depends on the trifurcation or bifurcation of lobar bronchi of 3D bronchial tree and the shape of carina in VB images. In this study, we tried to identify the segmental bronchial main ramification types, combining the two post-processing images of bronchial tree and VB, and study how to recognize them in transverse thin-section CT. We also attempt to display the unusual types of bronchi which can be seen in less than 5 percent of the total number and the sub superior segmental with 3D image.ResultsCombining bronchial tree,multiplanar reformation and CTVE,it is more accurate than with only one. The segmental bronchial ramification of the right lung bronchus was mainly classified into some types:upper lobe, trifurcation(60.3%,140/232);common stem of apical and anterior segmental bronchus(4.7%,11/232), common stem of apical and posterior segmental bronchi(19.8%,46/232),common stem of anterior and posterior segmental bronchi(12.1%,28/232); middle lobe, bifurcation (97.0%,225/232); trifurcation(2.76%,7/232); lower lobe, common stem of external lateral basal segment and anterior basal segment bronchus(50.4%,117/232); the basal trunk bronchus divides into five branches. (38.4%,89/232); common stem of internal lateral basal segment and anterior basal segment bronchus(9.1%,21/232).ConclusionThe combination of multiple post-processing of 16-slice CT poddes great superiority to classify the pattern of the right lung bronchus.ApplicationThis study intuitively shows the form and variation of every segmental bronchus of right lung by means of bronchus tree, and through MPR, every segmental bronchus is found and observed, which reveals the sectional CT display of the main branch types. This study classify the branch types of segmental bronchus quite correctly, help to find segmental bronchus even more accurately, which serves the demand of the precise locating of lesion in diagnostic imaging, and meanwhile, helps guide the pulmonary segmentectomy for patients with bronchiectasis or tumor, who are inadequate to pneumonectomy, even more precisely.
