Investigation And Analysis On The Condition Of Dalian Community Health Service Cost

Objective:By researching the cost, income and health services of public community health service centers in dalian, analysis their cost composition, and estimate the cost of the public health service projects which have been taken, in order to strengthen management and cost control, save health service resources, improve service efficiency and compensation mechanism of community health services and promote the sustainable development of community health services.Methods:Use field investigation to collect the basic condition of community health service center, and analyse the cost and its components by cost survey methods, and calculate the actual cost and standard cost of public health service project by the way of time distribution coefficient proposed by Cheng Xiaoming compared the compensation situation to find out the problems and make suggestions.Results:The proportion doctors comparing with nurses is 1:0.40 in Dalian community health service centres,which is higher than national average level. In the total cost, pharmaceutical trading costs and health service costs are basically at the same level, respectively 1530.69 million yuan and 1467.04 million yuan. In 6 categories cost, the highest is labor cost for 1065.07 million yuan, followed by depreciation of fixed assets 344.07 million yuan, and the two types take the proportion of 84.96%. There are 9 types 33 projects of public health service which have been carried out in community health service centers, basically encompassing the six kinds of functions, but their actual costs are all higher than their standard costs. There three ways to compensate community health service centers including government subsidies, pharmaceutical trading incomes and medical care incomes. Pharmaceutical trading incomes take 60.00%, government subsidies take 27.00% and medical care incomes take 13.00%, but in general 34.73 million yuan compensation gap still exists.Conclusion:The current economic development of our country is in transition, new medical health system reform is also carrying on warmly. The development of community health service shows great effects in people's livelihood. Health technicians in Dalian community health services centre have high quality in general, and the public health services which have been carried out are comprehensive, and government subsidies are higher than national level, which reflect good development of Dalian community health service. However, the compensation mechanism of Dalian community health service is not perfect.The compensation is insufficient to the cost and some community health service centres have compensation gap, meanwhile the government subsidies to public health services are not enough, either. The structure of health technicians is unreasonable, and the actual cost is higher than standard cost of public health services. Community health service institutions need to strengthen internal management, Optimize the configuration of human resources, mprove work efficiency, and make efforts to control costs.The gorvenment should establish the compensation mechanism to community health service based on cost analysis, increase subsidies according to reduced pharmaceutical trading income and the standard costs of public health services, strengthen cost control, promote the effective interaction formed between community health services and government support, to fundamentally solve the bottleneck of Dalian community health development.
