Objective:To investigate the influence of anti heparin/platelet factor 4 antibodies in thrombocytopenia and thrombosis for chronic renal failure patients undergoing hemodialysis.Methods: A total of 94 patients undergoing regularly hemodialysis in our hospital were selected in the time interval of February 2010 and October 2010. All the patients received unfractionated heparin(UFH)or low molecular weight heparin (LMWH) for the sake of anticoagulation during hemodialysis. The blood samples that platelet, hemoglobin, serum creatinine, serum urea, calcium and phosphorus were collected before dialysising at the same time that the blood samples of HPF4 antibodies were collected. Groups:According to the level of HPF4 antibodies high or normal, the patients were divided into two groups:the high HPF4 antibody group and the normal HPF4 antibody group.Results:(1) The mean platelet count in the high HPF4 antibody group was lower than the normal HPF4 antibody group(P<0.05).(2)The average levels of platelet, hemoglobin, serum creatinine, serum urea, calcium and phosphorus showed no significant difference in the high HPF4 antibody group and the normal HPF4 antibody group (P> 0.05).(3) The dose of unfractionated heparin and low molecular weight heparin showed no significant difference between the two groups (P> 0.05).(4) In high HPF4 antibody group, the risk of thrombocytopenia was significantly higher than in the normal group (P<0.05,RR=3.24,).(5) In high HPF4 antibody group, the risk of thrombosis was significantly higher than in the normal group (P<0.05, RR=2.67). (6) Between the two groups, there were no significant difference in the incidence of bleeding tendency (P> 0.05).Conclusion:For chronic renal failure patients undergoing hemodialysis,the average platelet count in the high HPF4 antibody group was lower than in the normal group. Moreover, in the high HPF4 antibody group, the risks of thrombocytopenia and thrombosis were significantly higher than in the normal group.
