Objective:Through observing the number change of CD3+Tcell, CD4+Tcell, CD8+Tcell, the expressions of CD4+HLA-DR+, CD8+HLA-DR+and the secretion of CD8+ Granzyme-B+ as well as the Treg cell quantity change in tubercular pleurisy patients'hydrothorax and peripheral blood to study the pattern change of a BTcell subsets and immunological pathogenesis in the patients who with Tuberculous pleurisy,then analyzeing the significance of that in developing tuberculosis.Methods:1.Collected 16 Tuberculous pleurisy patients'peripheral blood and hydrothorax and peripheral blood of10 normal control group at the same time.2.16 Tuberculous pleurisy patients'peripheral blood and hydrothorax were detected by flow cytometry(FCM) technology to analyzeing of the change of a BTcell subsets (CD3+Tcell, CD4+Tcell, CD8+Tcell,Tregcell) and the levels of that, meanwhile 10 normal peripheral blood is detected as control. Statistical analysis the difference and correlation of aβT lymphocyte cell subsets in patients who with pleurisy and control group.Results:1. Compared with control group, the population of CD3+Tcell, CD4+Tcell, CD8+Tcell in peripheral blood were greatly declined in tuberculous pleuritis patients(P<0.05), however, the number of CD3+Tcell, CD4+ Tcell in hydrothorax were obviously increased Compared with peripheral blood in tuberculous pleuritis patients (P<0.05). But the number of CD8+Tcell without changed in peripheral blood and hydrothorax (P>0.05), the number of CD3+ Tcell and CD4+Tcell were positively correlated in hydrothorax (r=0.602; P= 0.000,<0.05) 2. Compared with control group, the expression of CD4+HLA-DR+ CD8+ HLA-DR+ in patients peripheral blood were found remarkably rised in tuberculous pleuritis patients (P<0.05), and the CD8+Granzyme.B+ were notably droped (P<0.05). Compared with patients peripheral blood, the expression of CD4+HLA-DR+, CD8+HLA-DR+, CD8+Granzyme.B+ in hydrothorax were notably droped (P<0.05). the number of CD8+Tcelland CD8+Granzyme-B+ were positiveiy correlated in patients peripheral blood (r =0.502; P= 0.000,<0.05) 3. Compared with control group, the population of Treg cell in peripheral blood were greatly rised in tuberculous pleuritis patients(P<0.05), the number of Treg cell in hydrothorax were obviously declined compared with peripheral blood in tuberculous pleuritis patients (P<0.05).Conclusion:1.ThedistributionofCD3+Tcell,CD4+TcellandCD8+Tcell in the tuberclousisitis' peripheral blood and hydrothorax presented distinct, the number of CD3+Tcell, CD4+Tcell in hydrothorax is higher than cycle blood outside the patients; pointing out cellular immunity enhanced in hydrothorax, take CD4+Tcell as the principal cells.And the amount of CD3+Tcell, CD4+Tcell and CD8+Tcell, the ratio of CD4+T/CD8+T in patients'peripheral blood were reduced evidently, so the immunity cell's function in patients peripheral blood is lowly and dysfunction. 2.In tuberclousisitis'peripheral blood, CD4+, CD8+'s activating degree were stronger than hydrothorax. CD4+T, CD8+T's are have a hand in immune response.The number of CD8+T's influence the expression of CD8+Granzyme-B+ 3.The number of Treg cell in the tuberclousisitis'peripheral blood and hydrothorax are higher. It may takes a negative effects in tuberclousisitis' peripheral blood and hydrothorax.
