Objective: to discuss bronchoscopy of clinical application value in children's segmental pneumonia. Methods: Retrospective analysis of bronchoscopy's the diagnose and treatment results, includ 1123 children who had segmental pneumonia, which 707 cases of bronchoscopic treatment group, the conventional treatment control group is 416 cases , at First Hospital of Jilin University ,from 2007.07 to 2011.05 Results: Bronchoscopic treatment group 707 patients , the number is doing Bronchoscopy is 783 , bronchoscopy mainly shows mucosal edema, mucous membranes pale, granular hyperplasia, purulent changes, sputum formation, longitudinal folds, bronchial openings stenosis and so on,most cases have two or more morphological changes. The average duration of mucosal congestion and edema is (11.35±2.01) days, we can prove that it is the acute phase performance of segmental pneumonia, but the average duration of mucous membranes paleand granular hyperplasia is (19.97±3.71)days, we can prove that it is the chronic phase performance of segmental pneumonia. Microscopy showed purulent lesions,or the formation of sputum,prompted that the children were associated with bacterial infection, there were 203 cases whose endoscopic features were longitudinal folds, mainly prompted to mycoplasma infection. Treatment group patients underwent BALF bacterial, fungal culture, the positive results was 150 cases, the positive rate was 21.22%. Which G + cocci in 57 cases, accounting for 38%, G-bacilli in 73 cases, accounting for 48.67%, fungal infections in 20 cases, accounting for 13.33%. Bacterium Streptococcus pneumoniae infections was the most common, accounting for 25.33%, followed by Pseudomonas aeruginosa (8.67%) and Acinetobacter baumannii (8%). Compared with the conventional treatment group, bronchoscopic treatment group were significantly lower at the duration of fever, cough duration, the average length of stay, average hospital costs, imaging change recovery time, in addition,lung abscess , atelectasis , and other complications were significantly lower, P <0.05, statistically significant. Comparing bronchoscopic treatment group three subgroups of the duration of fever, cough duration, the average length of stay, average hospital costs, imaging change recovery time, the incidence of complications, (5 ~ 10d) group <(10 ~ 15d) group <(> 15d) group, P <0.05, statistically significant. Prompted that bronchoscopy of early implementation will help shorten the course and prevention complications. Conclusion: 1. Bronchoscopic observation of morphology and Bronchial alveolar lavage (BAL) is an important mean of the etiological diagnosis of segmental pneumonia.2. Diagnosis and treatment technique of bronchoscopy is an effective treatment method for segmental pneumonia. 3. Bronchoscopy of early implementation will help shorten the course and prevention complications.
