Abstract/Summary:
ObjectiveGiant cell tumor of bone(GCT) is one of the primary bone tumors which have strong invasiveness and high recurrence rate,insensitive to the chemotherapy and radiothe- rapy,sarcomatous change after radiation therapy.The chief histology of giant cell tumor is many well-distributed multinucleated giant cells which suffusion and hyperplasia can find on the background of stromal cells.Pathologic grading and radiology grading can't predict the tumor's biologic behavior correctly,which bring many problems,such as estimate invasiveness and select therapeutic schedule correctly.So it is important to identify a tumor specific marker which is signifinance in the design of an interventional procedure,guiding for control invasiveness effectively and decrease recurrence rate.Therefore,this experiment to study the roles of OPG and OPGL in development and progression of bone tumors.To analyze the expression localization of OPG and OPGL in giant cell tumor of bone and detect the correlation between their expression levels and invasiveness of giant cell tumor. To intend to establish foundation of molecular biology for bisphosphonate application in the clinical.Materials and methodsRetrospectively analyze 31 patients which were surgically treated by local excision in the Orthopeadic Department of General Hospital of Jinan Military Command from January, 2000 to August,2010.The clinical manifestations and changes of 31 cases giant cell tumor of bone(GCT);Analysing the preoperative X-ray manifestations in 31 cases of giant cell tumor of bone by using X-ray score system.The expression of OPG and OPGL in 31 cases of giant cell tumor of bone was studied by using S-P immunohistochemical method,to analyze the relationship between the expression levels and clinical-pathological features, Jaffe grading,Campanacci's grading,the recurrence and X-ray manifestations.ResultsThe level of the expression of OPG in MGC and STC of giant cell tumor of bone was 80.65% and 74.19% respectively.The positive expression rate of OPG in MGC was not related to the gender,invasive location,Jaffe grade,Campanicci's grade and X-ray manifest- ations(P>0.05);but related to the age, the prognosis and the recurrence (P<0.05);The strength of OPG positive expression in MGC was negative correlate to the prognosis (rs=-0.397,P<0.05),but positive correlated to the X-ray manifestations(rs=0.444, P<0.05); The positive expression rate of OPG in STC was not related to the gender, age,invasive location,Jaffe grade,Campanicci's grade,the prognosis,the recurrence and X-ray manifesta- tions(P>0.05),the strength of OPG positive expression in STC was negative correlate to the prognosis(rs=-0.390,P<0.05).The level of the expression of OPGL in multinucleated giant cells and stromal cells of giant cell tumor of bone was 41.94% and 67.74% respectively.The positive expression rate of OPGL in MGC was not related to the gender,invasive location,Jaffe grade,Campani- cci's grade,the recurrence and X-ray manifestations (P>0.05);but related to the age and the prognosis(P<0.05);The strength of OPGL positive expression in MGC was negative correlate to X-ray manifestations(rs=-0.511,P<0.05),but positive correlated to Campanicci's grade(rs=0.377,P<0.05)and the recurrence (rs=0.595,P<0.05);The positive expression rate of OPGL in STC was not related to the gender,invasive location,Campanicci's grade,the prognosis,the recurrence and X-ray manifestations(P>0.05),but related to the age and Jaffe grade(P<0.05),The strength of OPGL positive expression in STC was negative correlate to Jaffe grade(rs=-0.534,P<0.05).ConclusionsThere are OPG and OPGL expression between MGC and STC in the giant cell tumor of bone.The invasion of tumor cell was negative correlate to the strength of OPG positive expression in MGC,but positive correlate to OPGL,The experiment confirmed that MGC take participate in the process of osteolytic destruction of giant cell tumor of bone,so the bisphosphonate can regulate the effection between osteoblasts and osteoclasts by OPG/RANK/RANKL system,at the same time,it has positive significance as adjuvant therapy that decrease rate of the recurrence,control invasiveness, ameliorate limbs functi- on.
