Objective:DSA as the gold standard to evaluate the carotid artery ultrasound, MRA and CTA in the value of the diagnosis of cerebrovascular lesions. Comparison of MRI and SPECT to assess both the value of diagnosis of the actue cerebral infarction.Methods:1.conducted to collect clinical carotid ultrasound, cerebral MRA, CTA and DSA examinations brain patients with cerebrovascular disease inspection information and statistics of the various inspections and the detection of the number of vascular stenosis and vessel stenosis, and DSA comparison, studied: ultrasound, MRA and CTA diagnostic carotid and vertebrobasilar system sensitivity, specificity and coincidence rate; ultrasound, MRA, CTA diagnosis of carotid and vertebrobasilar system to compare stenosis; ultrasound, MRA, CTA diagnosis of cerebral vascular stenosis, occlusion of the sensitivity, specificity, consistent rate.2.the collection of clinical brain MRI and SPRCT been checked and were diagnosed as cerebral infarction, infarct location were calculated and compared, and the calculatedχ~2 value.Results:1 . Ultrasound diagnosis of carotid artery system sensitivity, specificity, compliance rates were 50.00%, 98.80%, 92.71%; diagnosis of vertebrobasilar system sensitivity, specificity, compliance rates were 81.48%, 97.74% , 95.00%. Diagnosis of carotid MRA sensitivity, specificity, consistent rate was 54.17%, 98.81%, 93.23%; diagnosis of vertebrobasilar system sensitivity, specificity, compliance rates were 85.19%, 97.74%, 95.63%. Diagnosis of carotid CTA sensitivity, specificity, consistent rate was 100.00%, 99.19%, 99.24%; diagnosis of vertebrobasilar system sensitivity, specificity, compliance rates were 100.00% and 98.00%, 98.33%.2.Ultrasound diagnosis of carotid artery and vertebral basilar artery stenosis comparison,χ~2 = 6.15, P <0.05,There was significant between the two differences. Ultrasound diagnosis of vertebrobasilar artery stenosis than carotid artery system reliability; MRA diagnosis of carotid artery and vertebrobasilar artery stenosis compared,χ~2 = 6.04, P <0.05,There was significant between the two differences. MRA diagnosis of vertebrobasilar artery stenosis than carotid artery system reliability; CTA diagnosis of carotid artery and vertebrobasilar artery stenosis compared,χ~2 = 0.48, P> 0.05, No significant between the two differences.3.Ultrasound diagnosis of cerebral vascular stenosis sensitivity, specificity, compliance rates were 65.23%, 98.39%, 94.03%; diagnosis of cerebral vascular occlusion of the sensitivity, specificity, compliance rates were 80.00%, 100.00%, 99.72%. MRA diagnosis of cerebral vascular stenosis of the sensitivity, specificity and accuracy were 69.57%, 98.39%, 94.32%; diagnosis of cerebral vascular occlusion of the sensitivity, specificity, compliance rates were 80.00%, 100.00%, 99.72%. Diagnosis of cerebral vascular stenosis CTA sensitivity, specificity and accuracy were 100.00% and 98.27%, 98.48%; diagnosis of cerebral vascular occlusion of the sensitivity, specificity and accuracy were 100.00%, 100.00%, 100.00%.4.MRI compared with SPECT, theχ~2 value is 22.53, p <0.001 between the two significantly different. SPECT for the detection of acute cerebral infarction was significantly higher than that of MRI.Conclusions:1.Color Doppler ultrasound and MRA diagnosis of vertebrobasilar system reliability higher than the internal carotid artery system; CTA diagnosis of carotid and vertebrobasilar no differencebetween the systems. DSA is the gold standard for diagnosis of brain disease,but its high cost,trauma, time-consuming, risky, not as the preferred method of diagnosis of cerebrovascular disease, treatment can be preferred; CTA is a micro- invasive examination, the accuracy of diagnosis of cerebral vascular stenosis than MRA and color Doppler ultrasound, lower than DSA; MRA is noninvasive, relatively inexpensive, but the noise, long time, diagnosis of cerebral artery stenosis higher than the accuracy of color Doppler Ultrasound, but less than CTA, DSA; Color Doppler ultrasound is convenient, safe and low cost advantages, can be used as a diagnostic screening method for cerebral vascular stenosis. 2.SPECT in the diagnosis of acute cerebral infarction is better than MRI in time, but MRI is better than SPECT in space.
