ObjectivesTo find out the prevalence of pneumoconiosis complicated with tuberculosis and the risk factors in three Iron and Steel Groups A ,B and C which are subordinated to Hebei Iron and Steel Group Co., Ltd.,in order to understand the situation and characteristics of pneumoconiosis complicated with tuberculosis and provide scientific criteria for the prevention of pneumoconiosis complicated with tuberculosis.Object and Methods1.Object Collected the occupational health surveillance data diagnosed by all of the recorded cases of pneumoconiosis (including deaths) which was in the prevention of occupational diseases of the construction of three Iron and Steel Groups.The cases of pneumoconiosis diagnosed according to national standards( the diagnosis of pneumoconiosis before 1986 used the national standard in 1963;after 1986 used GB 5906-86;after 2002 used GBZ 70-2002,after 2009 used GBZ 70-2009) and the diagnostic criteria of tuberculosis(comprehensive diagnosis of Chest X-ray examination, sputum culture and sputum smear examination, lung biopsy and clinical diagnostic treatment).Pneumoconiosis patients carried on film examination regularly per year and the active pulmonary tuberculosis cured was not as simple pneumoconiosis.2. Methods Inspected the occupational health surveillance data and fill out all items of "card of pneumoconiosis cases" of all confirmed cases of pneumoconiosis.New cases of workers exposed to dust of the Iron and Steel Group used face to face questionnaire.The content of questionnaire included name, gender, age, unit, type of work, length of service, type of pneumoconiosis, diagnostic time and stage, time and stage of pneumoconiosis complicated with tuberculosis , time of death, cause of death and so on.The database was built by Excel2003 and the data was analysed by SPSS13.0.Results1.The rate of pneumoconiosis complicated with tuberculosis in Iron and Steel GroupA,GroupB and GroupC were 12.89%,23.44%and 19.05%.2.Three Iron and Steel Groups have shown that the rate of pneumoconiosis combined with tuberculosis increased with the age of onset and the stage of pneumoconiosis and the working age increasing.For the types of distribution, Group A showed higher rate of pneumoconiosis combined with tuberculosis in drilling workers, casting foundry workers, refining (rolled) steel workers.The Group C showed higher rate in drilling and refining steel workers.With the years of exposure increased,the rate of pneumoconiosis combined with tuberculosis of Group A and Group C declined. And with the working age increased,the rate of pneumoconiosis combined with tuberculosis of Group A and Group C declined.3.Risk factors of pneumoconiosis complicated with tuberculosis which exposed to dust of iron and steel were observed to be the age of exposure(OR=1.643) , the years of exposure(OR=0.468) , the working age(OR=0.796),the incidence age(OR=1.467) and the stage of pneumoconiosis(OR=2.390).Conclusions1.The rates of pneumoconiosis complicated with tuberculosis in three Steel Groups under Hebei Steel Group was different.The group B was higher and group A lower.2.The rate of pneumoconiosis complicated with tuberculosis in Iron and Steel increased with the age of onset and the stage of pneumoconiosis and the age of exposure increasing.With the years of exposure and the working age increased,the rate of pneumoconiosis combined with tuberculosis declined.3.The risk factors of pneumoconiosis complicated with tuberculosis which exposed to dust of the Iron and Steel Group were observed to be the age and the years of exposure, the working age,the incidence age and the stage of pneumoconiosis. The risk of pneumoconiosis complicated with tuberculosis of different types of distribution in individual Iron and Steel Group were different.
