Objective: To evaluate the clinical treatment that adopts the method of straight wire technology,which is infused with Tweed—Merrifield's idea of orientated force,and the anchorage that is provided by micro-implant to the patient who was classified as complex adult Angle ClassⅡdivision 1.Method: Selecting a patient who was received on the 27th of February 2009 as an example from Affiliated dental hospital of Dalian Medical University.The patient has been diagnosed as dental Angle ClassⅡdivision 1,female,25 years,with a convex facial figure, incompetent lips with excess incisal display, distal relationship of bilateral molars,Ⅲ°overbite,12mm overjet,deep curve of Spee,perfect mid-line, and sporadic space among upper and lower dental arch. The anterior teeth of upper and lower jaw are labially torqued. After the extraction of certain teeth,the patient has received the treatment of combination of straight wire technology and Tweed—Merrifield's idea of orientated force.The patient has been analyzed in 34 representative measurements from clinical and cephalometric aspects that are including the methods of Peking University analysis, Tweed analysis, Wits analysis, Wylis analysis.Results: The treatment lasted for 26 months. The pre and post date comparison:(1)The relationship of the molars have been corrected from distal relationship to neutral relationship, the positions of SNA and SNB have hardly changed.The axes of the anterior teeth of the upper and lower jaws have been noticeably changed,overjet has been decreased by 9mm.The torque of the upper anterior teeth have been remedied,U1-SN has decreased by 14.56°,and the distance of U1-NA has been decreased by 13.55mm.The lower anterior teeth are currently upright,L1-MP has been decreased by 7.22°,and the distance between L1-NB has been shortened by 5.42mm.The angle that is formed by the axes of the upper and lower anterior teeth has been increased by 19.40°,the protrusion has been decreased. Meanwhile, anchorage control was ideal resulting in the distance of U6-Ptm unchanged.(2)The distance of N-ANS has been maintained, whereas the distances of N-Me and ANS-Me has been slightly increased.S-Go/N-Me has been decreased by 1.98%,which indicates a clockwise rotation of mandible,which is desired for the correction of the facial figure.(3)Soft tissue profile has been changed dramatically.ULP,LLP have been decreased 7.33°and 7.76°respectively.SnPg'-FH has been decreased by 4.01°,nasolabial angle has been increased. Nasal shape looks more define than before the treatment.Z angle has been increased by 11.29°,H angle has been decreased by 12.64°,mental region has moved forward , Pg'B'-FH has decreased by 27.16°with the positions of chin and lips more harmonious. The patient was satisfied with the result of the orthodontic treatment.Conclusion: During the period of time of concealed treatment that adopts the method of straight wire technology,which is infused with Tweed—Merrifield's idea of orientated force for the patients those who are classified as complex adult Angle ClassⅡdivision 1, combining the technology of micro implant each will negate the other's shortcomings and not only remedy the protrusion of the upper dentition, but also achieve the movement of the lower posterior teeth as a whole.This method will shorten the treatment time and the result will be more stable.
