Objective:Early detection of changes of intestinal barrier function(impairment of intestinal barrier function)when intra-abdominal hypertension occurred through testing diamine oxidase(DAO)and intestinal fatty acid-binding protein(I-FABP )in the peripheral blood.Methods:Fifty four healthy adult rabbits from New Zealand were randomly divided into 9 groups according to random digits table. The first group-control group,the second group-IAP ( intra abdominal pressure ) 15mmHg-30min, the third group-IAP15mmHg-60min , the fourth group-IAP15mmHg-90min, the fifth group-IAP15mmHg-120min , the sixth group- IAP 20mmHg-30min, the seventh group-IAP20mmHg-60min , the eighth group-IAP20mmHg-90min, the ninth group-IAP20mmHg-120min.The rabbits were deprived of food and water about 12 hours before establishing an animal model. Applying the nitrogen gas pneumoperitoneum , three catheters of pressotransducer, 16G plastic catheter with side hole and nitrogen gas cylinder were connected with three-ways tube respectively. nitrogen gas was influxed slowly in order to establish model of intra-abdominal hypertensive rabbits(The speed of boosting pressure was 2mmHg/5min), Intra-abdominal pressure was maintained homeostasis. Femoral venous blood was collected in each group,and double antibody sandwish enzyme-linked immunosorbent assay (ELISA) was used to determine the content of DAO and I-FABP. One point five centimeter of small intestine, about 20cm far from Treitz ligament , was cut off and observed under light microscope.Results:The contents of DAO, I-FABP in rabbits serum increased gradually along with the increase of intra-abdominal pressure and duration. The contents of I-FABP from the first group to the ninth group were 0.05, 0.07, 0.10, 0.43, 0.55, 0.62, 0.81, 0.91, 1.19(unit:μng/ml)in turn.The contents of DAO from the first group to the ninth group were 0.11, 0.15, 0.32, 0.60, 0.94, 1.22, 1.48, 1.87, 2.25 (unit:μng/ml)in turn. Pathological sections of the small intestine through HE stain were observed under light microscope: The small intestine was not damaged and the structure of villi were basically integral in the first group. The intestinal mucosa was damaged lightly, the structure of villi were basically integral, the top of villi of epithelium dropped partially in the second group. The part dropping from villi of epithelium developed to bottom gradually, epithelial interspace widened and cell disintegrated in the third group. The top of villi of intestinal mucosa dropped and atrophied gradually in the fourth group. The villi were damaged seriously and parts of structures of villi disappeared in the fifth group. The cell edema and submucous hyperemia occurred in the sixth group. The top of villi were damaged and capillaries of lamina propria exposed in the seventh group. A large quantity of villi were damaged and a large quantity of capillaries of lamina propria exposed the eighth group. The damages of lamina propria of the small intestine were most seriously the ninth group. And differences exist statistical significance.Conclusion: Along with the increase of intra-abdominal pressure and duration, the contents of DAO and I-FABP in blood of intra-abdominal hypertension rabbit increased and intestinal villi pathological damage aggravated gradually. Therefore, the determination of contents of DAO and I-FABP in peripheral blood can reflect the damage of the structures and functions of intestinal mucosa resulted from intra-abdominal hypertension.
