Objective:Myocardial protection is the method to reduce heart surgery during myocardial ischemia and reperfusion.And the myocardial protection method for infant are always focuses on hot in clinical and scientific discussion, the purpose of this topic is to explore for the method through cold self-biood cardioplegia to MMP-2 for myocardial ischemia and reperfusion.Methods:60 infants suffered from ventricular septal defect (the data VSD) with extracorporeal circulation had been selected.30 cases, were used cold self-blood cardioplegia perfusioning,and 30 cases were used crystal heart cardioplegia perfus-ioning. Before the aorta blocked instantly,and after opening the aorta 1 minute,5 minute respectively,each 1ml blood from coronary sinus and surrounding venous had been collected. MMP-2 concentration was assayed with ELISA. Postoperative car-diac index (CI), ventricular arrhythmia happen and positive inotropic drug dependen-ce had been monitored.Results:In same group MMP-2 concentration after opening aortic blocking aorta significantly increased than before the aortic blocks (P<0.05) Between two groups, the MMP-2 concentration before the aortic has no singnificant defference (P>0.05).); The aorta lmin after opening the heart, the cold self-blood cardioplegia 5min the cold self-blood cardioplegia MMP-2 concentration of a crystal heart cardioplegia MMP-2 concentration of reduced significantly (P< 0.05). the cold self-blood cardioplegia group began monitoring ventricular arrhythmia significantly lower incidence of crystal heart cardioplegia group, a statistically significant difference (P<0.05). Positive inotropic drug use of the cold self-blood cardioplegia for positive inotropic drug dependence situation was obviously lower than crystal cardioplegia group. a statistically significant difference (P< 0.05). Postoperative 1,12, 24 and 48h, two groups cardiac index (CI) gradually increases, but the cold self-blood cardioplegia group than a crystal heart cardioplegia group, was more elevated statistically significant difference (P<0.05).Conclusion:MMP-2 concentration markedly increase in infant proceeding ventricular septal defect(VCD) reparied with extracorporeal circulation. Cold self-blood cardioplegia can reduce the release of MMP-2 early stage,and has the favorable protection for infant proceeding ventricular septal defect(VCD) reparied with extracorporeal circulation.
