Hepatitis B virus belongs to the hepatotropic DNA virus family .Human hepatitis B virus (HBV) basically cause damage to liver leading to infectious diseases, chronic hepatitis, cirrhosis, and 80% of the major cause of hepatocellular carcinoma., Everybody can be suscetible to HBV infection.Patients with acute hepatitis B, chronic hepatitis B patients and hepatitis B surface antigen (HBsAg) carriers are sources of infection. Hepatitis has a world-wide prevalence, with China having a high prevalence rate. China has a high incidence rate of HBV infection causing serious infectious diseases, which are harmful to society and family and high disease burden. It is estimated that hepatitis B virus carriers in China is about 9300 people. According to the Notional Diseases Reporting System (NNDRS),in 2007 hepatitis B patients had the highedst total number of reported incidence.Objective: The objective of this study was to analyse the epidemiological situation oHepatitis B in monitoring sites of Luyuan district during the period 2006~2008, understand the major risk factors of hepatitis B in Lvyuan district . Compare and Contrast the results of the NNDRS and the results of monitoring unit of incidence of hepatitis B cases and categories, analysis the accuracy of the NNDRS report, calculate the actual incidence of hepatitis B monitoring. Based on the results of the monitoring unit of incidence of hepatitis B ,identify deficiencies and offer reasonable suggestions, and further provide perfect scientific basisfor further the monitoring systerm of hepatitis B .Methods: From the NNDRS reported cases of hepatitis B identified true cases hepatitis B suitable for surveillance and epidemiological investigation. Blood specimen collected, laboratory testing done at the provincial center of disease control laboratory. The results were reviewed to determine the final diagnostic classification.Results: During the period 2006 to 2008 ,the average annual incidence rate of hepatitis B in Lvyuan was 31.8/10 million. In 2006 the incidence rate was 42.82/10 million, in 2007 the incidence rate was 42.10/10 million,and in 2008 the incidence rate was 40.93/10 million. The city average annual incidence rate was 32.06/10 million while the rural areas had average annual incidence rate of 31.28/10 million. The male to female ratio was 2.20:1. The age group 30~40 year had the largest proportion of incidence. Based on occupation, manual workers had the largest number of cases, with a percentage of 23.92%. Clinical diagnosis of hepatitis B cases and the provincial CDC classification diagnosis has a low consistent (Kappa = 0.173). Hospitals doing five tests case detection of hepatitis B had a high consistency with the result of the privencial CDC (50.94% ) while Hospitals doing only one test case detection of hepatitis B(HBsAg), had a low consistency with the result of the privencial CDC (24.05%) .This difference in consistent was statistically significant (χ2 = 17.82, P <0.05). Hospitals testing for anti-HBc IgM has a low consistency when compared with provincial labs (κ= 0.297). The most common risk factor for hepatitis B is exposure to trauma during barbing ( 46.7%).Conclusion: From 2006 to 2008 the average annual incidence rate of hepatitis B in Lvyuan declined each year. The average annual incidence rate in the city is higher than in rural areas.Manual workers had the highest proportions of cases of hepatitis B, medical personnel constitute the minimum number of cases.The NNDRS diagnostic classification of cases has a low consistency when compared with privencial CDC. The actual incidence rate of hepatitis B in Lvyuan is lower than the one reported by the NNDRS in same period.Family history of hepatitis B, Barbing trauma history, trauma history during beauty make-up,and history of tooth extraction are major risk factors of hepatitis B in Lvyuan.
