ObjectiveTo assess the effect of exogenous basic fibroblast growth factor to the repair of the stabbing annulus fibrous.Methods18 new Zealand white rabbits , despite of gender, were exposured the L3/4,L4/5,L5/6 intervertebral spaces from the anterior-lateral of right abdomen; there were 54 intervertebral discs. The anterior-lateral of annulus fibrous(AF) was stabbed by trepan whose diameter was 2 millimeter, if you saw the nucleus pulposus(NP) overflows from annulus fibrous, you must stop stab the AF. The 18 rabbits were divided into 3 groups averagely and random. The first group( k group) were just stabbed by the trepan. Put the gelfoam into the cleft which was on the annulus fibrous , which was looked as the second group(m group).Inject 0.1ml bFGF into the gelfoam that have been putted into the cleft which was looked as the third group( b group). Then we marked the interverbral disc which were intervened by us. We executed 2 rabbits every two weeks from the second week after surgery; took the x-ray from the lateral of lumbar vertebral;got DHI; then take the intervertebral disc which were intervened; measured the volumn of the proliferation which lied at the position of cleft,made HE chromosome of the sample to observe the alteration of morphology and counted respectively the number of fibrocyte and chondrocyte which consist of the granulation tissue.Results1.We could find proliferation which was similar to the cone lied on the cleft of annulus fibrous despitting of time and group, which is covered by a thin fibrous- cartilaginous tissue . The volume of the 4th week was bigger than the one of the 2nd week; there was no different between the 4th week,s and the 6th week,s. At the same time ,the volume of group b was bigger than others; there was no different between group k and group m.2.There were two sections taking the space of the outer annulus fibrous: the cartilage proliferation occupied the middle of the outer AF ,which comes from nucleus pulposus's chondrocyte-like cells, granulation tissue lied between the cartilage proliferation and the broken end of the AF, we could find chondrocyte in which from the 2nd week,b group had more chondrocyte. We found NP on the inner of some AF at the point of cleft, others nothing. There was little fibrocyte in the broken end of AF. A cap which was consisted of fibrocyte and chondrocyte covers the surface of proliferation and link the two broken end of AF.3.The number of fibrocyte of the 2nd week is the most and the 6th week is the least ;the number of the chondrocyte was just opposite; the number of fibrocyte and chondrocyte of the b group was more than the others at 2nd,4th week; the number of fibrocyte had no different among the 3 groups at the 6th week but the number of chondrocyte was more. 4.The DHI had no different among the 2nd , 4th ,6th week, at the same time there was no different among 3 groups.Conclusion1.The repair of stabbing annulus fibrous is confined to the outer of AF, the inner of AF is disability to repair.2.There are two section take the space of the outer annulus fibrous: the cartilage proliferation occupy the middle of the outer AF ,which come from nucleus pulposus's chondrocyte-like cells, granulation tissue lie between the cartilage proliferation and the broken end of the AF, the granulation gradually transform to cartilaginous tissue,bone tissue;a cap which is consisted of fibrocyte and chondrocyte cover the surface of proliferation and link the two broken end of AF.3.The exogenous bFGF could promote the proliferation of granulation tissue,cartilaginous tissue which lie in the position of stabbing annulus fibrous, promote the granulation tissue get into cartilaginous tissue and bone tissue; but the bFGF could not contribute to the change of intervertebral space.
