Diabetes, a common disease, is a group of metabolic diseases which characterized by increasing of chronic blood glucose level. According to WHO, there were 30 million diabetics around the world in the late 70s early 80s of 20th century, but the number had reached at 150 million in the mid-90s.The number of diabetics would double in 2025.Our surveys showed that the prevalence of adult diabetics was 1% between 1979 and 1980 and 2.5% between 1994 and 1995. It is estimated that there are approximately 30 million patients in China now, ranked NO.2 around the world. In recent years, with the improvement of living standards, population aging, lifestyle changes, the incidence of diabetes is increasing rapidly. Vulvovaginal candidiasis is a common vaginitis in diabetic patients with highest incidence. Also, it is a vaginitis which is difficult to cure and has high recurrence rate. Thus, it is worth to explore the mechanism of vulvovagianl candidiasis in diabetes.It has been reported that TGF-β1 as an immunoregulatory factor which can play a role in inhibition of cell-mediated immune response was increased in the murine model of estrogen-dependent vulvovagianl candidiasis. Also , it has been found that TGF-β1 play a role in diabetes and its complication. Therefore, we presume that TGF-β1 play a role in vulvovagianl candidiasis in diabetes.In this study we investigate whether TGF-β1 play a important role in vulvovagianl candidiasis in diabetes to explore the mechanism of it. PartⅠEstablishment of diabetes and vulvovaginal candidiasis in diabetes rat modelObjective: To establish diabetes rat model and vulvovaginal candidiasis in diabetes rat model for further study of the expression of TGF-β1.Methods: After 12h of fasting, 26 female SD rats, weighing approximately 200g, received a single 60 mg/kg intraperitoneal injection of streptozotocin. A week later, 10 rats were randomly selected from the success of diabetes rat models as the group of vulvovaginal candidiasis in diabetes (DC group, n =10) and were inoculated with spores of candida albicans (5×10~8). Blood glucose was detected by blood glucose meters and the infection of candida was observed by applying microscope and culture.Result: After a week single intraperitoneal injection of streptozotocin, blood glucose was greater than 16.7mmol / L, and the rats had more drinking, strong urine odor and anorexia. At day 7 after inoculated, a large number of spores and amount of pseudohypha were observed in vaginal fluid.Conclusion: Diabetes rat model and vulvovaginal candidiasis in diabetes rat model were established successfully.PartⅡThe expression of TGF-β1 in vagina of diabetes and vulvovaginigtis candidiasis in diabetes rat modelObjective: To detect the expressions of TGF-β1 in vaginal tissues of diabetes rats and vulvovagianl candidiasis in diabetes rats.Methods: 36 female SD rats, weighing approximately 200g, were randomly divided into diabetic group(D group,n=26) and control group(C group,n=10). After fasted 12h, the D group received a single 65mg/kg intraperitoneal injection of streptozotocin. The C group received equal amount of citrate buffer alone. A week later, 5 rats from each group were killed and the vaginal tissues were obtained. The other diabetic rats were further randomly divided into the group of vulvovaginal candidiasis in diabetes (DC group, n =10) and indomethacin treated group of vulvovagianl candidiasis in diabetes (DI group, n =10). The other rats in C group were used as the group of vulvovaginal candidiasis in non-diabetic group (CC group, n =5). Three groups were inoculated with spores of candida albicans (5×10~8). DI group was treated with indomethacin (2m M, 100ul) about 6 days at one day after inoculated, while giving equal amount of sterile saline to the other two groups. Vignial tissues were obtained at day 7 after inoculated. The infection of candida was observed by applying microscope and culture. The levels of TGF-β1 were detected by using ELISA.Results: More spores and pseudohypha were observed in DC group. No pseudohypha but spore was observed in DC group. No spore was observed in CC group. CFU in DC group was significantly higher than in DI group following CC group. The levels of TGF-β1 in D group were significantly higher than C group (P<0.05). The levels of TGF-β1 in DC group were significantly higher than CC group (P<0.05).The levels of TGF-β1 in DI group and DC group have no difference (P<0.05).Conclusion: TGF-β1 plays an important role in the development of vulvovaginal candidiasis in diabetes rat.
