Objective To investigate the cause of infertility and evaluate the clinical diagnostic value of laparoscopy in diagnosis of infertility.Methods A retrospective study in January 2005 ~ March 2010 in Ningxia Medical University Hospital, Analyze the relationship between pelvic disease and infertility in 253 female patients with laparoscopy and laparoscopic operation. (1)Calculate rate of success,positive examination,and incidence of complication of laparoscopy to evaluate efficacy and security of it. (2)Comparison of hysterosalpingography (HSG)and laparoscopy examination in the detection rate of oviduct; evaluation of laparoscopy in the diagnosis of oviduct.(3)Analyze the distributed feature of oviducal diseases of infertile patients.Results (1)Among 253 cases, 253 patients (successful rate of laparoscopy was 100%) were examined successfully; 242 of 253 cases (95.65%)had pelvic disease. There was no concurrent disease during the examination. (2)103 of 253 infertile patients received HSG before laparoscopy examination at most a year (198 fallopian tubes were examined totally). There are 101 patients'results are sameness, 51.01%. Fake positive rate of fallopian tube block by HSG is 24.75%. (3)Pelvic inflammatory disease, endometriosis and polycystic ovarian syndrome (PCOS)were major causes resulting in infertility, approximately 81.82%, 30.30% and 17.58% respectively. There were 88.89% patients with pelvic inflammatory disease and 26% with endometriosis presented fallopian obstruction,there were significant differences(P<0.05). Pelvic inflammatory disease in primary infertility and secondum infertility was approximately 34.09%, 63.64%. there were significant differences(P<0.05).(4)The success follow-up cases is 174, the pregnancy is 86.Conclusion (1)Hydrotubation with laparoscopy is feasible. Laparoscopy detection is one of the effective ways that checks fallopian tube currently.(2)Laparoscopy can observe wether uterus, ovary and oviduct were abnormal or not and there were adhesion with surrounding tissue, if the oviducal peristalsis became rigid and its function was restricted. By observing the Methylene blue through the fallopian tube, determine the normal situation in it or not.(3)Laparoscopy can get more exactly information about the pelvic disease than hysterosalpingography. Laparoscopy is the first choice to diagnosis pelvic cause of infertility and it can diagnosis definitely and shorten the therapy time.
