| Hepatocellular carcinoma ( HCC) is one of the most common and malignant tumor, with the fifth prevalence rate and the fouth death rates. In our country it also has become the second killer in all malignant tumour. With decades of unremitting efforts, the partial liver cancer patients have achieved long-term survival because of early detection and early diagnosis and comprehensive treatmen, but overall treatment effect still not acceptable. Its main reason is that HCC metastasis and recurrence rate is high, even in early hepatocellular carcinoma. Studies show that hcc's metastasis and recurrence rate is about 61.5% after five years'radical excision, even if small HCC recurrence rate was also amount to 43.5% . hepatocellular carcinoma. Transfer has become further improve HCC treatment effect the main obstacle. Therefore, exploring genes and the molecular mechanism of hepatocellular carcinoma metastasis, looking for effective prevention and control mean to be of the key for further improvement HCC treatment.Fudan university liver cancer institute and the U.S. national institutes of health NIH, applied of a gene 9180 containing cDNA microarray (gene chip) technology in whole genome range, and compared with 40 cases between with and without metastasis, and between liver metastases and primary tumor gene with differences the gene expression patterns, as to find out the candidate genes called osteopontin (OPN) which might be closely related with liver metastasis. OPN overexpression is closely related with the metastatic potential and cell lines in vitro invasion of HCC; OPN antibody can effectively restrain high metastatic potential HCC characterization of in vitro and in vivo invasive pulmonary metastases, It suggests that OPN might play an important role in HCC transfer processOsteopontin(OPN) is a kind of secretory phosphorylationg lucoprote protein, the center has a special RGD sequences (Gly Arg. J j Asp),which is certain kind of integrin (such as integrin ave3, ave5) and CD44 ligand, playing roles of cell adhension protein and cytokines. Under normal circumstances,OPN appear only in the bone, a small amount of normal tissue and other non-mineralized tissues, such as vascular, kidney, gallbladder, etc. It can be also detected the rotein expression in macrophages and activated T lymphocytes. OPN is regulated not only during development in the organization, but also in many diseases (eg, tumors), inflammatory immune response, vascular remodeling and so on. OPN has been found of overexpression in many of malignant tumors such as laryngeal cancer, ovarian cancer, breast cancer, colon cancer, pancreatic cancer, mesothelioma, bone tumor, and confirmed it playing an important role in tumor invasion and metastasis, angiogenesis, inhibition of withered death in processes [1-4]. Recent studies show that, OPN overexpression increased with tumor metastasis and recurrence. Expression of OPN in patients of metastatic breast cancer significantly increased the. Hotte, etc. found that elevated levels of serum OPN with bone metastasis of prostate cancer was positive , while with the survival rate negative. In hepatocellular carcinoma, opn specificly make interaction with cell adhesion related by RGI sequences and the "transfer factor", such as integrins, CD44, nitrous oxide, vascular endothelial growth factor receptor, macrophage migration inhibitory factor, matrix metalloproteinase protein, fibronectin, Rho and other interactions to promote cell chemotaxis, adhesion and migration, OPN's mRNA expression is relative with tumor size, differentiation, tumor recurrence and lowness of 10-year survival rate. In animal exPCRiments, OPN in the highly metastatic tumor cell appear high expression, a high level of OPN even stimulate benign tumors cells to transe into malignant metastatic phenotype. After using antisense technology to reduce cells expression of OPN , the cell survival rate in soft agar decline, so as to the rate of nude mice tumor formation and the ability of distant metastasis . Recent studies have shown that, OPN can interact with HGF, EGF, VEGF and some other growth factor and receptor ion,or by activating the expression of certain proteases such as uPA, MMPst in tumorigenesis and transfer;. These findings prompted OPN played an important role in tumor invasion and metastasis, but the mechanism remains unclear, pending further study.Based on the theory and exPCRiment foundation above, this study is respectively to use fluorescent quantitative polymerase chain reaction method (RQ - PCR) and immunohistochemical method to detect HCC tumor tissue and cancer by contrast organization of express osteopontin mRNA and osteopontin protein, so as to clarify the expression of opn level in HCC; and further research relationship between expression of the OPN of mRNA and protein levels and clinical pathology;Generlly to clarify its important position invasion and metastasis in liver cancer.even to clarify its important role in liver metastasis and transefer. |
