Objective:1.The application Value of Spatio-Temporal Image Correlation (STIC) in the prenatal diagnosis of fetal heart,2. Compared with the diagnostic rate and accuracy rate between 2DE and STIC, Summarize the ultrasonographic feature of STIC on abnormal fetal heart structure, Evaluation STIC for diagnose of the fetal CHD.Methods:1. Acquire heart volumes using 4D ultrasonography with STIC in 240 normal fetuses between 20 and 31 weeks of gestation, then analyze the data to study the display rates of standard planes and Record the time of Fetal direct contact with ultrasonic. Compare the display rates of standard planes with 2DE and STIC.2.Acquire heart volumes using 2DE and STIC ultrasonography in 22 abnormal fetuses between 22 and 32weeks of gestation, Compared with the diagnostic rate and accuracy rate between 2DE and STIC, analyze and concludes abnormal fetal heart image feature then compared with the results of two-dimensional ultrasound after the birth or autopsy.Results:1. the time directing contact with ultrasonic to fetal of STIC is much less than 2DE (p<0.001)2.After using STIC process the data sets of the 240 fetal hearts, the display rates of stomach, Double ventricular axis,4 chamber view, LVOT,RVOT,3VT, Large-artery axis, DA, AA, Vena cava atrial are respectively98.75%,99.58%,97.08%,95.42%,96.67%,92.92%,89.58%.95.00%,96.25%,90.83%, and the display rates using 2DE echocardiography are 100%,100%,98.33%,97.08 %,97.92%,96.67%,92.08%,86.25%,90.83%,81.25%respectively. The display rates of stomach, Double ventricular axis,4 chamber view, LVOT, RVOT,3VT, Large-artery axis between STIC and 2DE ultrasound is with no significant difference (p>0.05), and for the display rates of DA(p=0.001<0.05), AA(p=0.016< 0.05)and Vena cava atrial (p=0.002<0.05), STIC is higher than 2DE ultrasound significantly.3.2DE diagnoses 21cases in 22 abnormal heart fetuses (21/22), diagnostic rate 95.5%;STIC diagnoses 22cases in 22 abnormal heart fetuses (22/22) diagnostic rate 100%; the diagnostic rate of 2DE and STIC has no significant difference (p=l>0.05) 2DE detects 35 lesions (35/53),accuracy rate66%; STIC detects 48 lesions (48/53),accuracy rate 91% the accuracy rate of lesions between 2DE and STIC has significant difference (p=0.002<0.05), and Using STIC can help to display the position, shapes and sizes of lesions directlyConclusions:1.STIC technology can reduce the time of fetus contacting with ultrasound and display the standard planes of fetal heart more easily and conveniently,So the difficulty of examination is cut down, and it is good for the popularity of fetal echocardiography.2.In the 240 normal cases, the display rates of AA,DA and Vena cava atrial, STIC is higher than 2DE ultrasound significantly.and for the display rates of stomach, Double ventricular axis,4 chamber view, LVOT,RVOT,3VT, Large-artery axis between STIC and 2DE ultrasound is with no significant difference.3.STIC Can display some cardiac structures which cannot show in common 2DE, SO it provides more information, evidence and broaden diagnosis ideas for the echocardiograph diagnosis of fetal CHD, the accuracy rate of STIC is higher than 2DE, it will be more and more valuable for clinical.
