| PurposeTo analyze the clinical effections of ultrafiltration technology in cardio which was used to treat for infants with congenital heart disease, explore the effection of conventional ultrafiltration, modified ultrafiltration and continuous ultrafiltration used to treat cardiopulmonary bypass in infants, in order to provide a basis and method for better prognosis after infant cardiac surgery.MethodsNinety children with congenital heart disease were selected from June.2006 to June.2010, who were given cardiothoracic surgery in the Fifth Affiliated Hospital of Zhengzhou University which body mass were under 13kg. Thirty cases were randomized in three groups. Systemly recorded and analyzed the general situation, three group in children with operation transit flow, and the blood potassium level when different super filter technology and cardiopulmonary bypass stop, hematocrit (HCT), hegmoglobin (Hb), when vascular activity drug application in three group after the operation, when auxiliary application of respiratory machin, after operation pericardial longitudinal diaphragmatic of introduction flow, and operation period blood products colloid dosage.ResultsWhen compared with age, body mass, disease distribution,) cardiopulmonary bypass time among three groups, the difference was of no significant statistically (P>0.05).As for filtering water, concentrated blood indexes of HCT(L/L) and Hb(g/L):when cardiopulmonary bypass stopped, HCT Hb level of conventional ultrafiltration group(28.31±1.59/95.00±9.00) is higher than that began(24.03±3.62/80.60±15.00)(P<0.05); when ultrafiltration stopped, the HCT, Hb level of the continuous ultrafiltration group and modified ultrafiltration group(33.90±1.35/33.05±1.15), (128.00±7.80/123.00±7.00) were higher than that of the machine stopped (27.25±2.03/26.98±1.92). (90.00±7.00/88.00±8.00) (P<0.05).As ultrafiltration was stopped, the HCT and Hb level of the continuous ultrafiltration group, modified ultrafiltration group (33.90±1.35/33.05±1.15). (128.00±7.80/123.00±7.00)was higher than that of conventional ultrafiltration group(28.31±1.59/95.00±9.00) (P<0.05), and the HCT and Hb level of continuous ultrafiltration group was above the modified ultrafiltration unit (P<0.05).All three methods(conventional ultrafiltration, modified ultrafiltration and continuous application of ultrafiltration)focusing on postoperative mechanical ventilation time (30.5±4.4/15.9±4.0/13.1±4.2) h, application of vasoactive drugs time(4.1±0.4/2.5±0.5±2.2±0.4)d, and peri-operative blood product amount of colloid(185±25.7/112±19.5/101±14.3)ml,which reflecting the organs function recovery:continuous ultrafiltration group and modified ultrafiltration group is superior to conventional ultrafiltration group (P<0.05).continuous ultrafiltration group better than the modified ultrafiltration group (P<0.05).In reflecting the coagulation function of postoperative pericardial mediastinum amount:three groups of the difference was not significant statistically (P>0.05).ConclusionsAll three methods(conventional ultrafiltration, modified ultrafiltration and continuous application of ultrafiltration) during cardiopulmonary bypass heart surgery in infants have concentrates the blood and reduce systemic interstitial edema;Continuous ultrafiltration was better than conventional ultrafiltration and modified ultrafiltration, might do more conducive to the recovery of heart and lung function after cardiac surgery in infants. |
