The most common clinical symptoms and signs of Portal hypertensive patients are the splenomegaly and hypersplenism.In recent years, we found that splenomegaly was not caused by congestion of spleen, the most important reason is increasing blood volume and the cellular elements of spleen. Changes in the structure of spleen play an important role in Changing splenic function and promoting Portal hypertension. Tissue factor (tissue factor, TF) and vascular endothelial growth factor (vascular endothelial factor, VEGF) play an important role in the vascular regeneration,organization construction and damage repair etal. TF and VEGF are important to effect splenic Vascular lesions and cell components.ObjectiveTo study the expression and distribution of TF, VEGF in the spleen of cirrhotic portal hypertensive patients.To analysis the mechanism of TF and VEGF in splenomegaly,hypersplenism and splenic high power cycle, And to explore the expression of TFand VEGF in the spleen has relationship with portal hypertensive patients clinical features.MethodsSpecimens of spleen (42cases) from cirrhotic portal hypertensive patients underwent modified sugiura procedure served as experiment group, normal splenic tissues (15 cases) served the control group. The distribution and expression of TF and VEGF in splenic tissues was studied by immunohistochemistry SP method.To analysis the clinical characteristics relationship and its correlation.Results1. The TF expression is difference in the experimental group and control group was statistically significant (P<0.01), the expression of TF in spleen of cirrhotic portal hypertension patients was not related to Child-Pugh,preoperative upper gastrointestinal bleeding and preoperative esophageal stomach bottom varices degree (P>0.05).The different expression of TF is statistically significant in the third degree (mild, medium, sever) splenic tissue of the splenomegaly and hypersplenism (P<0.05).2. The VEGF expression is difference in the experimental group and control group was statistically significant (P<0.01), the expression of VEGF in spleen of cirrhotic portal hypertensive patients was not related to Child-Pugh,preoperative upper gastrointestinal bleeding and preoperative esophageal stomach bottom varices degree (P>0.05).The different expression of VEGF is statistically significant in the third degree (mild, medium, sever) splenic tissue of the splenomegaly and hypersplenism (P<0.05).The expression of TF and VEGF is positively correlation in the splenic tissue of cirrhotic portal hypertensive patients(r=0.490,P=0.001).Conclusions1. The expression of TF and VEGF was mainly Seen in the splenic tissue of cirrhotic portal hypertensive patients.It suggests that TF and VEGF Participate the splenic organization construction of cirrhotic portal hypertensive patients and play an important role in promoting splenomegaly,hypersplenism and high power cycle of spleen.2. The expression of TF and VEGF in the splenic tissue of portal hypertensive patients is basic identical, It suggests that TF and VEGF is positively correlation in the splenic construction and fuction of cirrhotic portal hypertensive patients.
