Objectives:To detect the serum level of IGF-1, Aβ1-40and Diabetes-related indicators in the elderly diabetes with dementia. To analyse the risk factors of elderly diabetes concurrent dementia.Methods:The patients were selected from December 2009 to July 2010 in the Department of Geriatrics of our hospital.30 elderly diabetes with dementia were group A,30 non-diabetic patients with dementia were group B,30 elderly diabetes with non-demented patients were group C,30 healthy elderly people were group D. The group A was excluded congenital, drug, severe cardiovascular and cerebrovascular diseases and other reasons due to non-diabetic dementia. The elderly diabetes was diagnosed by 1999 WHO diagnostic criteria. The diagnosis of dementia according to the American Diagnostic and statistical manual of mental disorders, fourth edition, DSM-IV diagnostic criteria for dementia.The fasting blood of patients were collected in the morning, the IGF-1 and Aβ1-40 and diabetes-related indicators were detected. IGF-1 and Aβ1-40 were tested by enzyme-linked immunosorbent assay. The blood glucose was tested by the glucose oxydases law, FINS were determined by radioimmunoassay, glychemoglobin (HbAlc) was determined by high pressure liquid ion-exchange chromatography law, other indicators completed by the automatic biochemical analyzer. The insulin resistance index (HOMA-IR) was calculated by HOMA model.Results:1. Comparison of the clinical dataComparison of the clinical data in the 4 groups, with or without diabetic urine, hypertension, coronary heart disease, etc, the difference was significant (P<0.05,).2. Comparison of clinical and biochemical indicatorsComparison of the serum level of FBG,2hBG, HbAlc, FINS, HOMA-IR in the 4 groups, the difference was significant (P<0.05). Comparison of the serum level of LDL-C, HDL-C, TC, TG in the 4 groups, the difference was not significant (P> 0.05).3. Comparison of Serum IGF-1,βamyloid (Aβ) levelsThe serum levels of IGF-1 in A group were significantly lower than the other three groups (98.86±17.17), B and C were not significantly difference (118.22±33.91 vs 117.08±28.57), D group was the highest (147.46±40.82), the differences of the four groups were significant (P<0.05). The serum levels ofβ-amyloid protein (Aβ) in A group was the highest (0.893±0.237), D group was the lowest (0.431±0.063), B group and C group were in the middle, but the group B is higher than the group C (0.657±0.096 vs 0.582±0.091), The differences of the groups were significant (P<0.05).4. Analyse the variables of diabetic patients with dementiaTo analyse the variables of diabetes-related disease and clinical indicators about elderly diabetes with dementia. Put hypertension, coronary heart disease, FBG,2hBG, HbA1c, FINS, HOMA-IR, TC, TG, HDL, LDL, IGF-1, Aβand other variables into logistic regression equation, found the risk factors of elderly diabetes concurrent dementia including hypertension, coronary heart disease, HbA1c, FINS, HOMA-IR, IGF-1, Aβ.Conclusions: Elderly diabetes with dementia is closely related to hypertension, coronary heart disease, serum IGF-1, Aβ1-40 level, HbA1c, FINS, HOMA-IR, Above 7 factors are the risk factors of the elderly patients concurrent dementia.
