Hypertensive disorder complicating pregnancy (HDCP) is a commom and specif ic disease,which only belong to pregnancy and be to one of the renson in cau sing pregnant and lying-in women and newborn to die.This disease often happen in the twentyth week around,and,patient often see the doctor because they lie under unre nsonation,headache,eyes blurred,Edema of lower extremity and so on.,test the blood pressure is higher than 140/90mmHg,the urine appear protein, what is more serious is that,patient can happen systemic-organ exhaustion to lead to death. In accordance with the severity,we can put this disease into five types:hypertensive disorder complicating pregnancy,mild or severe preeclampsia,eclampsia,hronic hypertension complicated with eclampsia,pregnancy complicated with chronic hypertension.In my country epidemiology-study,we could see the morbidity of HDCP was 9.4%, mild-preeclamsia is 77.7%, severe-preeclampsia was 22.3%. the mortality of pregnant and lying-in women was 18.9/1 hundred million in city.The characteristic of HDCP is universality,heritability and the high mortality, so more and more people began to study nosogenesis to find the best treatment prescription.There are a lot of hypothesizes about nosogenesis,but the most focus on ischemia,inflammation,oxidative stress,immunity,heredity etc,but in recent,with the increasingly apoptosis were regarded,study on this were more and more,the relation on trophocyte-apoptosis and HDCP is becoming hotspot nowadays.HSP70 is called stress protein,which common consist in eukaryon and prokar yote,can produce in condition of high temperature,inflammation,ischemia and infections.lt be regarded as the most content and most pessimistic and most impor-tant in HSP-family.It play role in cytoprotection,cell proliferation,apoptosis,anti-oxidation and etc in addition to known as "molecular chaperones". P38MAPK is one of the member in MAPK-family,its main effect is cell signaling, in addition,in cell in-vasio,natherosclerosis,apoptosis. Caspase3 is an protein enzyme of apoptosis,which is the most important in Caspase-family and the performer in apoptosis cascade reacti -on,its activation indicate the occurrence of apoptosis.The experiment can analyse the pathogenesis of HDCP form the apoptosis,and discuss the effect of HSP70,P38MAPK,Caspase3,the purpose is providing the theory evidence to HDCP-treatment. Objectives:The experiment was designed to observe the morphology of apoptosis and respectively test and compare the expression of HSP70,P38MAPK,Caspase3 in normal pregnancy,mild preeclampsia,severe preeclampsia in placenta tissue, using the immunol histochemistry SP and image analysis.Analyse the relation of HSP70,P38MAPK,Caspase3 and placenta cell apoptosis, which can cause HDCP.,further,discuss the pathogenesis of HDCP.1. object of reseach:placenta tissue.60 cases were selected from July 2008 to July 2010,who admitted to The Second Affiliated Hospital of Zhengzhou.All pregnant were divided into two groups, as follows:(1) control group:20 cases of normal pregnancy were stochastic selected who delivery in The Second Affiliated Hospital of Zhengzhou.(2) HDCP:75cases, hypertensive disorder complicating pregnancy group: 20cases,mild preeclampsia:20 cases,severe preeclampsia:30cases.The selected standard of subject investigated should obey these requirement singleton pregnancy, non-complication on pregnancy(for example:GDM, chronic internal or surgery medicine complication,etc), neopathy(for example:PROM, intra-uterine infection,FGR,placenta praevia etc), select the way of termination of pregnancy is uterine-incision delivery.The standard of diagnosis and classification should refer to the textbook< obstetrics and gynecology >,which punlished in renmin Medical Publishing House,the7edition,lejie chief editor.2. The study method:(1) All specimen adopt the method of HE and immunol histochemistry stain and image analysis to exam placenta tissue the average grey level of HSP70,P38MAPK,Caspase3.(2) the result of experiment were analyzed by SPSS 17.0 and P<0.05 were defined as statistical significance.Results:1. Under the microscopic examination,swelling villi,fibrinoid necrosis of villi, hyperplasia of syncytial knots and cytotrophoblast can be found.However,these microstructural changes are rarely found in the normal placenta.2. clinical general data comparison:Compare normal pregnancy control group with HDCP groups (hypertensive disorder complicating pregnancy mild-preeclampsia,severe -preeclampsia),we found that the age and gestational weeks within these groups were non-statistical significance (P>0.05)3. the expression of HSP70 in placenta tissue:Comparing normal pregnancy control group,HDCP groups were higher (P<0.05),and present the trend of ascend with the more and more serious; meanwhile, the expression of HSP70 in mild-preeclampsia groups were higher than HDCPgroups P<0.05, severe-preeclampsia groups were higher than mild-preeclampsia groups, P<0.05, the difference was regarded as statistical significa -nce.4. the expression of P38MAPK in placenta tissue:Comparing normal pregnancy control group,HDCP groups were higher (P<0.05),and present the trend of ascend with the more and more serious; meanwhile, the expression of P38MAPK in mild-preeclampsia groups were higher than HDCPgroups P<0.05, severe-preeclampsia groups were higher than mild-preeclampsia groups, P<0.05, the difference was regarded as statistical significance.5. the expression of Caspase3 in placenta tissue:Comparing normal pregnancy control group,HDCP groups were higher (P<0.05),and present the trend of ascend with the more and more serious; meanwhile, the expression of Caspase3 in mild-preeclampsia groups were higher than HDCPgroups P<0.05, severe-preeclampsia groups were higher than mild-preeclampsia groups, P<0.05, the difference was regarded as statistical significa -nce.Conclusions:1. the normal pregnancy trophocyte cell exsit apoptosis,but HDCP had higher obviously and apoptosis cell increased with the more and more serious,which point out that the incresea of apoptosis participate in the occurrence of HDCP.2. HSP70,P38MAPK,Caspase3 all expressed in placenta tissue,and the expression higher with the more and more serious,which point out that they played an important role in HDCP.3. HSP70 could inhibite apoptosis,but it gone up in HDCP,which point out that it could inhibite apoptosis in disease,besides,as a stress protein,played a protectiveness role in disease.
