BackgroundThe Parkinson's disease is a common neurodegenerative diseases, its clinical manifestations can be divided into movement and non-movement symptoms symptoms.Movement symptoms has been well known because of its performance, but the non- movement symptoms were not given sufficient attention.Depression is the most common non-exercise symptoms in Parkinson's disease. PD patients with depression is the an important Dysfunction reason,and get more and more attention now.With the subthalamic nucleus (Sub thalamic Nucleus, STN) deep brain stimulation (Deep Brain Stimulation, DBS) widely used in PD treatment, the efficacy of PD dyskinesia has been clearly affirmed.In recent years, research gradually focus on the non-motor symptoms of STN-DBS in patients PD.The depression after STN-DBS in PD patients is inconclusive. The improvement of depressive symptoms on STN-DBS PDD patients ,as well as postoperative adjuvant treatment of antidepressant drugs applied to achieve the best effect, is an important issue faced of neurosurgery ObjectiveTo study the effects of paroxetine in treatment of Parkinsonps disease with depression after deep brain stimulation of the bilateral subthalamic nucleus .Methods38 cases of PDD patients were randomly divided into three groups, All patients were selected for STN-DBS,after surgery ,control group with nothing antidepressant, themedication group with therapied by Paroxetine,another group with placebo.Assessment was performed before and 1, 2, 3 months after surgery. Depression was evaluated using the Hamilton Rating Scale for Depression (HAMD).ResultsAfter surgery, The depressive disorders such as anxiety, despair and agitate were alleviated , the placebo group's effect outgo the control group(P<0.05), the themedication group's scores of HAMD decreased significantly than control group and placebo group(P<0.05).ConclusionBilateral STN-DBS improves the depressive symptoms in PD patients, coordinate with antidepressant have better effects in treating PDD patients. .
