Efficacy Comparison Of Bricker Operation With Ureterostomy In Bladder Cancer Patients

Objective: To compare the Recent and long-term Curative efficacy of Bricker operation and ureterostomy,and investigate these clinical significance.Methods: A total of 26 patients after radical cystectomy , including 16 cases with Bricker operation(Group A) and 10 cases with ureterostomy(Group B) , were analyzed retrospectively. Collect data by using the questionnaire,survey content include:degree of education,economic conditions,intraoperative conditions, postoperative situation,complications, quality of life.Results: In Bricker operation group,there were 1 cases lung inflammation, 2 cases incision infection, urine fistula in 1 in postoperative recent, were cured. After a month of the operation, mild hydronephrosis was found in 2 case , Hydronephrosis was disappeared after 2 month of the operation, Skin creates mouth infections was found in 3 case.In Ureterostomy group,there were 2 cases lung inflammation, 2 cases incision infection in postoperative recent, were cured.Light, medium, heavy hydronephrosis was found in 6 case in Ureterostomy group, 4 case Hydronephrosis was disappeared after 2 month of the operation, 2 cases of patients with mild hydronephrosis after 2 month repeatedly check reduce, But 1 cases of heavy hydronephrosis persist for four years, Skin creates mouth infections was found in 6 case, 4 cases have been healed after by anti-infection treatment, 2 patients suffered repeated infected more than 1 year. No operative death in 26 cases. Follow-up of 3 months to 7.5 years, 10 deaths, 2 patients died of cancer metastasis, 1 patients died of lung infection in Bricker operation;3patients died of cancer metastasis, 1 patients died of lung infection, 1 patients died of coronary heart disease, 1 patients died of kidney failure in ureterostomy, Average survival 26.2 months, 1 patient had the longest survival 6 years.There were significant difference in operation time,volume of blood , Postoperative hospitalization days,postoperative decannulation days and the postoperative complications between Bricker operation and ureterostomy,the latter was slightly better than the former(P<0.05);but for comparison of postoperative complications,the former was obviously better than the latter(P<0.05);The quality of life of the patients in the former was more improved than the latter(P<0.05).Conclusion: Bricker operation is a good means of urinary diversion,is the preferred method to radical cystectomy patients.Ureterostomy is a reasonable selection in some patients.
