objectivesComparison of gestational diabetes by measuring the pregnant women, pregnant women with abnormal glucose tolerance and normal pregnant women, serum retinol binding protein 4 (RBP 4) and adipose tissue phosphatidylinositol 3 kinase (PI3K) content and its relationship with triglyceride, total Cholesterol and other associated clinical and biochemical indicators, clearly in the pathogenesis of gestational diabetes mellitus and insulin resistance factor in this fat RBP4, PI3K role.Methods120 subjects selected from March 2009 to May 2010 in Dalian Maternity Hospital to be produced in pregnant women, subjects were divided into three groups, of which 40 normal pregnant women, pregnant, women with abnormal glucose tolerance was 40 (75gOGTT only one at or above normal), 40 patients with gestational diabetes mellitus. According to BMI,48 patients were divided into normal weight and overweight in 72 cases..All pregnant women at 24-28 weeks pregnant gestational diabetes screening line. SPSS10.0 Accountancy software applications according to groups were compared and analyzed.Results (1) gestational diabetes mellitus group, retinol binding protein 4 levels are highest in the normal control group, retinol binding protein 4 levels are lowest, P<0.001, so the differences were statistically significant; serum RBP4 in overweight pregnant women is higher than its level of normal weight pregnant women, P<0.05 was statistically significant.(2) PI3K activity in the GDM group was 83.21%, PI3K activity is in GIGT group 88.46%, the two groups were found compared with the control group, P<0.01 were considered statistically significant result.(3) Fasting insulin levels in GIGT group is highest in the NGT group is the lowest, more significant differences were found (P<0.05), fasting serum insulin levels in the three groups of pregnant women for the [NGT: 12.58±2.314 GIGT:19.01±4.538 GDM:17.33±3.924 (mIU/1)].(4) HOMA-IR in GIGT group and GDM groups were higher than the normal group, with a statistically significant difference (P<0.05), and GDM groups GIGT group and no statistically significant difference, HOMA-IR in the NGT, GIGT, GDM women in the three groups were:2.44±0.352,3.81±0.948,3.91±0.543.(5) For the RBP4 with this experiment other indicators of linear correlation analysis obtained:RBP4 levels and pregnancy HOMA-IR was positively correlated (r= 0.358 P<0.05), and pregnancy BMI was positively correlated (r= 0.458 P<0.001), and fasting blood glucose also positively correlated (r= 0.352 P= 0.005), and come with the gestational age, TG, TC no correlation (P>0.05); PI3K activity in the GDM group, negative correlation in the third trimester of pregnancy weight, body mas index in late pregnancy and HOMA-IR (r=-0.52, r=-0.68, r=-0.77, P all<0.01); PI3K activity is negatively correlated GIGT group in the third trimester of pregnancy weight, body mass index in late pregnancy and the HOMA-IR (r=-0.61, r=-0.59, r=-0.81, P all<0.01). By multiple stepwise regression analysis:BMI and fasting blood glucose during pregnancy were independently associated with RBP4.Conclusions(1) GDM patients with insulin secretion in the body than women with normal pregnancy, although increasing, but the increase in the amount of increase is far offset by the degree of insulin resistance, that is dominated by insulin resistance, GDM may be the direct cause of disease in this. Compared with normal pregnant, women found that patients with GDM and gestational impaired glucose tolerance and insulin resistance were higher body mass index, showing that increased insulin resistance during pregnancy one of the factors is obesity, weight gain.(2) P13K as a signaling molecule involved in insulin signaling pathway in the biological role of signaling molecules and GIGT in GDM patients, and led to the formation of IR.(3) RBP4 in overweight, GDM group were higher, and RBP4 and HOMA-IR, BMI and fasting blood glucose was positively correlated, suggesting that insulin resistance in pregnancy and the incidence of GDM is inextricably linked with the involvement of RBP4.
