ObjectTo investigate the main factors contributed to the collagen fiber by observing the expression of CTGF in the tissue of bladders wall of different model rats, which can provide the evidence for prevention and treatment of the clinical patients who had the disease of neurogenic bladder.MethodsFirstly, different animal models were established.70 female Wistar rats,3-4 months old with the average weight of 200-250g. All the rats were randomly divided into three groups:bladder outlet obstruction group 30 rats (Group A).. nerve damaged group 20 rats (Group B) and the control group 20 rats (Group C). In the former group, animal were given the surgery of partial bladder outlet obstruction. For the second group, we had an operation of spinal cord (L6-S1) transection and vesicovaginal fistula to set up an animal model of pure nerve damaged model. The remaining 20 rats which opened the dura mate of spinal cord only without injuring the spinal cord were as a control group. After the surgery, intraperitoneal injection of gentamicin with the does of 5000U/kg and the frequence of 2 times per day was given in the next 3 days. On the tenth and the thirty day,10 rats were randomly selected from every group, undergoing urodynamics and bladder harvest to weigh it. All the bladder specimens had to receive a series process of HE staining and immunohistochemistry, so that we can observe the changes and the expression of CTGF in bladder wall by the help of microscope. Among all handled groups, and at the same time controlled group, applied variance analysis, when P<0.05, it is statistics meaning.Result:1 The general stateGroup A:8 rats died within the first week after the operation.3 rats were failed to make the model. For the group B,2 rats died, the other rats were well, urine can be smoothly drained by vesicovaginal fistula. There was not bad influence for hindlimbs. All the rats were good at everything.2 Urodynamics(1) Group A:On the 10 and 30 day after the operation the bladder capacity, pressure of detrusor were significantly more than the group C (P<0.05). While the compliance were all less than the group C.(2) Group B:On the 10 and 30 day after the operation, the bladder capacity, compliance were were significantly more than the group C(P<0.05). but the pressure of detrusor were less than the group C(P<0.05).3 The weight of specimensThe weight of bladder in the group A were significantly more than the group C on the 10 and 30 days after the operation. For the group B, the weight of bladder were not significantly more than the group C (P>0.05).4 HE dyeing (1) Group A:the bladder wall were thicker than the normal, smooth muscle layer were the most obvious. Smooth muscle cells were hypertrophied. More etracellular matrix can be observed in submucosa and muscle bundles, which were significantly more than the group C. The nucleus of smooth muscle were bigger and stain were stronger than the normal ones. During the period, more inflammatory cells had invaded in the tissue.(2) Group B:the bladder walls were not thicker or thinner than before. But it could be observed that the smooth cells were disorganized. Inflammatory cells were also seen in every layers of the wall.(3) Group C:The tissue were normal, smooth cells organized in order. Inflammatory cells were less in every layers of the wall than the group A and B.5 The immunohistochemistry of CTGF(1) Group A:Immunostaining for CTGF was easily seen in layers of smooth muscles and transitional epithelium, which was stronger than the group C. A lot of staining particles were located in the cells. After 10 days,30 days positive rates were 72.2%,75%, staining grading compared with the control group (P<0.05), a statistically significant.(2) Group B:smooth muscles and transitional epithelium were stained to be weak yellow. Little staining particles were seen in the cells. After 10 days,30 days, the positive rates were 55.6%,88.9%, compared with control group statistically significant。(3) Group C:Mucosal layer of smooth muscle cells and mild jaundice can be seen, no abnormal deposition of brown granules, after 10 days,30 days positive rates were 20.0%,30.0%Conclusion1 The operation of spinal cord (L6-S1) transaction was a good way to make a model that can simulate the disease of neurogenic bladder, which can enhance the survival rate. 2 The high press of bladder was the key factor that contributed to significantly lower bladder compliance. The denervation of bladder could increase bladder compliance in the short term. 3 It could concluded that both the high press and denervation of bladder were the factors of increasing the CTGF in the bladder tissue, maybe these two factors were important reasons to promote bladder fibrosis...
