ObjectiveTo discuss the correlation between Thyroid Hormones and C-reaction Protion, Pulmonary function test in Patients with Acute Exacerbation of Chronic Obstructive Pulmonary Disease to explore the dependablity pathophysiology of COPD and its effect of diagnosis and clinical significance on pulmonary function in order to provide theorial evidence for reasonable diagnosis and appraisal prognosis treatment in COPD patients.Methods1. object:1.1 AECOPD group:50 cases of COPD, diagnosed according as the COPD guide Published in 2007, were enrolled in the study. They did not receive nutritional support and they have no other diseases as can influence organism metabolismand nutritional status. Such as active tuberculosis,malignant tumor,hyperthyroidism,diabetes and Liver function or kidney functional disturbance and other wasting disease.pulmonary function test verificate inconvertible airway obstruction. Serious cases cannot complete pulmonary function test were not enrolled this study. nearly appear cough. cough up phlegm,lose ones breath and so on, one or more than one items obviously aggravate, accompany or nonaccompany fervescence, symptoms continue more than two days, beyond usually changes, and require chang virgin regulation treatment plan. In addition may appear unweel,agrypnia,drowsiness,lassitude,athymia and mental aberration and so on.1.2 Catabasis group were enrolled in study, cough, cough up phlegm, lose ones breath and gasp for breath symptom stable or almost disappear for 1 month along.1.3 Healthy control group:20 healthy people were enrolled in study after the evenlation of history, physical inspection, auxiliary examination and without systemic infection in reneent 2 months.2. Measure C-reaction Protion and thyroid hormones in blood serum:8 ml vein blood was collected from all subjects in the morning and separated serum through centrifuge 15 minutes with a speed of 3000rm/in, after standing for 30 minutes, we measured the level of C-reaction Protion by immunoturbidimetry and we measured the levels of T3, T4, TSH by radio immunoassay.3. Pulmonary test:All the subjects received pulmonary function test, the main indexes were as follows:MVV%, FEV1%, FEV1/FVC%.4. Analysis dataSPSS13.0 statistic software were used to to analysis all the dataes, Analysis of variance was used among three groups. the average of all the samples used t test, the relation of variables were calculated by linear correlation analysis, a P value of<0.05 was considered statistically significant.Results1. Thyroid hormones in blood serum: The difference of T3 level in blood serum was statistically significant ((F=95.56 P<0.01) in triinterclasses, T4,TSH levels was not statistically significant (Respectively F=3.21,2.46, P>0.05) of group comparison.Comparing with the healthy control group the level of T3 in blood serum of AECOPD group was lower significantly(P<0.01).2. C-reaction Protion in blood serum:The difference of CRP level in blood serum was statistically significant (F=140.77 P<0.01) in triinterclasses.Comparing with the healthy control group,catabasis group, C-reaction Protion was higher in AECOPD group, and the difference was statistically significant (P<0.01).3. The levels of thyroid hormones and C-reaction Protion and their relationship to pulmonary test:By the analysis of linear correlation, The level of T3 in blood serum of AECOPD group was positivly correlated with MVV%,FEV1%,FEV1/FVC% (Respectively r=0.9621, p<0.01; r=0.1325, p<0.05; r=0.1234 p<0.05); and even more negatively with C-reaction Protion (r=-0.8785, p<0.01), the C-reaction Protion was negatively correlated with MVV%,FEV1%,FEV1/FVC%(Respectively r=-0.8663, p<0.01; r=-0.3862, p<0.05; r=-0.3979 p<0.05).Conclusion1.The levels of T3 in blood serum was concerned with Chronic obstructive pulmonary disease hypoxemia,malnutrition, participate the pathogenesy.2. C-reaction Protion value of Chronic obstructive pulmonary was advanced accompany with exacerbation of inflammation, after anesis obviously degraded, non-anesis continue in high level.3. The levels of T3,C-reaction Protion of Acute Exacerbation of Chronic Obstructive Pulmonary Disease were dependable, and were concerned with pulmonary function index of AECOPD group (MVV%, FEV1%, FEV1/FVC%), the both can reflected seriousity of COPD, together used to assistant diagnose,treatment and observe prognosis have higher clinical valuation.
