Background and objectiveAutoimmune liver disease(AILD) is a group of immune-mediated liver injury.According to clinical manifestations, biochemical, radiological and histopathological features, AILD can be simply divided into the main types of autoimmune hepatitis, autoimmune hepatitis(AIH) and in biliary damage and cholestasis predominant primary biliary cirrhosis (PBC) and primary sclerosing cholangitis(PSC), as well as any of the three diseases overlap between the syndrome. These diseases can be manifested as severe liver disease, and may progress to cirrhosis. At present, there are no drugs to cure. The currently it is accepted that genetic susceptibility combined with environmental factors is a causal factor in AILD.AILD related domestic research is focused on autoimmune own characteristics, such as the identification of autoantigens and the immune response against these antigens correlated with disease manifestations. The triggers of autoimmunity is not clear, the mechanism of injury is also poorly understood. There was a report showed that urinary tract infection history is one of the risk factors of PBC This study investigated retrospectively 100 cases from AILD patients and 39 cases from Chronic Hepatitis B(CHB) with urinary tract infection situation, preliminary discussion the relationship between AILD and chronic urinary tract infection. MethodsThe patients including 44 cases of AIH-1,27 cases of PBC,29 cases of AIH- PBC overlap syndrome and 39 cases of CHB were detected and follow-up the urine-RT and urine bacterial culture.Results44 patients who were from 100 cases of patients with AILD in the hospital were checked out urinary tract infection, accounting for 44.0%.Among these patients, there are 17 cases of AIH-1,15 cases of PBC,12 cases of AIH-PBC overlap syndrome. While there are 7 patients with CHB were checked out urinary tract infection, accounting for 12.4%. Among 51 patients with urinary tract infection, there are 27 patients with AILD have recurrent urinary tract infection. The patients with CHB only have one case of that.Conclusion1.Urinary infection rates in AILD group was significantly higher than that in the control group, so we speculate that urinary tract infection was associated with AILD.2. Urinary tract infection rates in PBC group was higher than other groups.4 cases of PBC patients with abnormal urinary tract imaging.It suggests that urinary tract infection as a risk factors of PBC, induced the genetic susceptibility to disease.
