Objective:The model of Endometriosis (EMT) in Wistar rat was established by self-endometrium transplantation, the effect of compound E'zhu capsule(CRCC) on the EMT model were investigated on histomorphology and the level of Micro vessel density(MVD) in eutopic and ectopic endometrium tissues.meanwhile, its therapeutic effects were also investigated on Vascular Endothelial Growth Factor(VEGF) and Endostatin(ES) in serum and peritoneal fluid of EMT model, all of those were done to study the therapeutic mechanism of CRCC from the angiogenesis perspective and to provide more theoretical and experimental proof for clinical application.of CRCC.Methods:The models of Endometriosis (EMT) in 45 female healthy Wistar rats were established by self-endometrium transplantation without testing estrous cycle.4 weeks after transplantation we made the second laparotomy to select right rats according to the inclusion and exclusion criteria.and then randomly assigned into four group:the hign-dose of CRCC,low-dose of CRCC, gestrinone group and the model control group.meanwhile,we select ten rats as the narmal control group and ten rats as the sham operation group.after 4 weeks'treatment, we made the third laparotomy to check the bulk and weight of transplant, to observe the pathomorphological changes with light microscopes, and to evaluate the expression of MVD in eutopic and ectopic endometrium tissues through immunohistochemical staining,then to measure vascular endothelial growth factor (VEGF) and endostatin (ES) content in serum and peritoneal fluid of EMT rats by using ELISA technology, experimental results are analyzed by SPSS17.0 statistical software, the comparisons before and after having drugs is processed by matching sample t-test, The comparisons between multiple groups are processed by completely random variance analysis, the comparisons between two groups are detected by Student—Newman—Keuls(SNK)method. the condition of P< 0.05 is considered to have statistical significance; P< 0.01 is considered to have obviously statistical significance.Results:1.The second laparotomy (4 weeks after modeling)confirmed:under the method of establishing the model of EMT by self-endometrium transplantation without testing estrous cycle, achievment ratio was78%.The endometrial impla nts in the rats became cystic structures that were composed of endometrium endometrial gland and stroma.the macroscopic and histological appearance were consistent with endometriosis. No significant difference was found in the volu mes of grafts between model control group and each CRCC therapy group on the second laparotomy(P>0.05),treated for 4 weeks, The volume of each CRC C therapy group on the third laparotomy was differently reduced compared wit-h the second Laparotomy(P<0.01),and was significantly reduced compared with the model controlgroup(P<0.01),the weight of grafts in the CRCC groups was significantly reduced compared with the model control group(P<0.01),No signi-ficant difference was found in the weight of grafts between each CRCC thera-py group and gestrinone group.2.From the histological point of view,ectopic endometrial tissues show different degrees of atrophy in treatment groups compared with the model control group,with the number of glands becoming less and the area becoming narrow, covered epithelial becoming thin, meanwhile epithelial cells show low columnar or flat or even loosely arranged and interstitial cells was becoming less, the number of blood vessels have diminished, no significant difference was found in HCRCC group on eutopic endometrial gland number, size and glandular epithelial cells and stromal cells compared with normal rats,3.MVD in eutopic endometrium has no significant difference in all of groups (F 2.019, P=0.096),MVD in ectopic endometrial tissue has significant difference in all of groups(F=19.967, P=0.000). MVD in ectopic endometrial tissue was higher in model control group than that of normal control group(P<0.05).MVD in ectopic endometrium decreased in HCRCC group and gestrinone group compared with the model control group(P<0.01), no significant difference was found between HCRCC group and gestrinone group (P=0.887). No significant difference was found between LCRCC group and model control group (P=0.110)4.the expression of VEGF in serum and in peritoneal fluid of model group was higher than the normal control group (P<0.01); the expression of VEGF in serum of the treated group was lower than the model control group P<0.01), while no significant difference was found among the treated groups (P>0.05). the expression of VEGF in peritoneal fluid,both in HCRCC group and in gestrinone group were lower than the model control group (P<0.01), no significant difference was found between the two group(P= 0.895).the expression of ES in serum was significantly higher in model group than the normal control group (P= 0.023); there was a downward trend, when we compare the expression of ES in serum in the treatment group with the control group, but there was no significant difference (P> 0.05).the result of testing the expression of ES in peritoneal fluid is meaningless. VEGF/ES in the serum and peritoneal fluid in model control group were higher than normal control group (P<0.05)and higher than HCRCC group and gestrinone group (P<0.05),there was no significant difference compared with the normal control group (P>0.05). no significant difference was found between HCRCC group and gestrinone group (P>0.05)Conclude:1.CRCC may inhibits the development of endometriosis and reduce the size and weight of graft, reduce abdominal adhesions, improve the pathological situations of ectopic endometrial tissue, the effect of HCRCC group is most obvious.2. MVD of EMT rat both in ectopic endometrium and in eutopic endometrium were higher than normal rats,high dose of CRCC can reduce the expression of MVD in ectopic endometrium of EMT rats,but has no significant effect on the expression of MVD in the eutopic endometrium.3. imbalance of angiogenic regulatory factors exist in serum and peritoneal fluid of EMT rats.mainly expresses for.increased VEGF levels, increased VEGF/ES value. CRCC can reduce the VEGF levels and the VEGF/ES value in peritoneal fluid and serum of EMT rats hign dose of HCRCC has the most Obvious effect.tere was no significant difference between HCRCC group and normal control group,indicating CRCC can not affect the normal angiogenesis.
