Objective:Optimal right ventricular (RV) pacing site in patients referred for permanent cardiac pacing remains controversial. The study was done to compare hemodynamic effect of permanent RV apex (RVA) vs RV septal (RVS) pacing.Methods:61 patients diagnosed as sick sinus syndrome with indication of pacemaker implantation were divide into two groups(36 patients in RVS group and 25 patients in RVA group). The baseline characters of two groups were compared. The effect of the different pacing site to LVEF and pacing parameter were evaluated.Result:The baseline characters, including age. gender, height, weight, hypertension, diabetic mellitus. coronary artery disease between RVS group and RVA group were no difference (P>0.05). The hemodynamic parameters during RVA pacing were significantly worse than preoperative parameters. In contrast, the hemodynamic parameters during RVS pacing were significantly better than preoperative parameters.Conclusion:There were significantly differences in hemnamic parameters between the RVS group and RVA group. RVS pacing could improve hemodynainic effect through keeping normal ventricular activation sequence and biventricular contraction synchrony.
