Migrant worker is the inevitable outcome of the reform and opening-up and modernization of China. In recent years, the size of migrant workers is increasing with a geometric growth rate. They bring strong labor force for the urban society and make a great contribution for the construction and development of cities. But most Migrant workers engage in dirty, dangerous, and demanding (so-called'3D') work, and their living and working environment is also very bad, nearly 85% of migrant workers working and living in such environment where there exists dust, toxic gases and liquid, noise, and safety hidden hazards that their health faces a serious threat. At the same time, migrant workers'sense of medical security and health insurance is under-established. They are called "marginal groups".This study is based on the survey of the situation of migrant workers in the five administrative areas of Changsha. This paper discusses the protection of migrant workers' health and medical insurance, and analyzes the health status and medical security conditions of migrant workers. The research finds that:(1) Health problem of migrant workers has not only ignored by the national system, work units, but also ignored by the migrant workers themselves, with weak health awareness and deficient health knowledge. It can be seen from the survey that migrant workers suffer from various diseases, health threat, and serious occupational hazards, furthermore, they generally take such passive attitudes as "tolerating" and "delaying" in face of diseases. (2) Owing to a lack of security knowledge and rights awareness, the rate of migrant workers' participation in urban medical insurance is low, and they are excluded by social system and work units. (3) The participation rate in urban medical insurance and work injury insurance is different for migrant workers of different gender, occupation, income, and education level, but the difference of this rate is not obviously related to the nature of business.At present when there is a consensus to construct a harmonious society, the government should speed up the reform of medical system and its attached systems to provide a substantial systematic foundation for the health of migrant workers. Furthermore, during the development of businesses, in addition to the pursuit of economic profits, the health and medication of migrant workers should be positively valued as well. Last but not least, migrant workers themselves should transform their traditional conceptions, actively striving for the resources of health security, and for getting rid of the situation of being disregarded on the aspect of their health and being excluded on the aspect of their medication.
