BackgroundWith the rapid development of the society, more and more seniors are in our life, therefore,the diseseas of age-related fragility fractures has become a worldwide public health hazard, it has brought tremendous suffering and financial burden for patients and their families.The orthopedic surgeon should not be limited to the provision of clinical surgical treatment for fractures of the program, they should consider the long-term and comprehensive arrangement for the patients, and be engaged actively in the prevention of brittle fracture of activities, they also take preventive measures for the benefit of the public. Therefore, how to prevent age-related fragility fractures has become increasingly major challenges for orthopedic surgeon.Fragility Fracture is the Osteoporotic fracture due to the brittle bone. It always happened at home or with little trauma, or when fall.It is common at spine, hip and wrist.The Traditional Chinese medicine stressed that"treatment aiming at its root causes", osteoporosis is "root" (Ben)and various secondary types of fragility fracture is "tip"(Biao). To control the occurrence of various types of fragility fracture,we must pay attention to the basic-primary osteoporosis prevention and treatment.There aren't records about"osteoporosis" in Chinese ancient medical books, according to its clinical manifestations and symptoms, primary osteoporosis belong to "bone atrophy","Gubi" category in traditional Chinese medicine. Its pathogenesis is 'faintness of kidney",which is generally accepted.with the further research, it is need re-understanding for basic pathogenesis of primary osteoporosis.Osteoporosis treatment with Chinese medicine has made significant achievements,our research apply Huo xue bu shen formula which promoting blood circulation and removing blood stasis, nourishing liver and kidney.Exploring the effect of formula for early intervention and the ability of anti-fracture in ovariectomized rat osteoporosis model to supply experimental basis for further research.Thoughts"Treating diseases before it occurs" is important principle for Traditional Chinese Medicine. In this study, under the thinking of "Treating diseases before it occurs",we gave drug intervention immediately when experimental animals were removing ovarian, it responsed the advantages of treatment in Traditional Chinese Medicine.PurposesTo further understand the basic mechanism of primary osteoporosis through literature. To study the effect of Huo xue bu shen formula on bone mineral density and bone biomechanics to ovariectomized rats;to evaluate the effect of Huo xue bu shen formula on morphology; to study application of ultrasound in experiment.Materials and methods65 female Wistar rats are used, with 10 months old,280±10g-weighted. Female Wistar rats were randomly divided into there groups:normal group (n=20), model group (n=22), test group (n=23). The model of postmenopausal osteoporosis was established through ovariectomy in rats.After anesthetized by 0.3% sodium pentobarbital injection,the normal contrast group was administered sham operation and other groups ovariectomy. The osteoporosis rats were treated with Huoxuebushen decoction the normal rats as standard control,and the model rats as model control. After 12 weeks treatment, histomorphological changes were observed with HE,Masson staining method. Then we determined the parameters of BMD, biomechanical experiments, and ultrasound scanning.The experimental data is expressed by (x±s). The statistic software is SPSS 13.0, the comparison among groups was performed by simple factor analysis of variance with LSD inspection or Dunnett-t inspection. P<0.05 means statistic diversity. Result(1)The pathogenesis of primary osteoporosis is deficiency of kidney and blood stasis.(2) By observing hair color, appetite, activity conditions, there were no significant differences in each group. Four rats dead accidently in process of raising.(3)There were no significant differences in the weight of three groups before experiment; there were significant differences between test group,model group and normal group(p< 0.01);Huo xue bu shen formula hadn't significant effect to the weight of ovariectomized rat.(4)Bone mineral density (BMD) in lumbar spine:to campare with normal group,there was significantly decreased in BMD of model group(P<0.01) .It was showed that BMD and bone mineralization were significantly decreased after removing ovarian.To compare with normal group, BMD of test group hadn't significant difference.It was showed that Huo xue bu shen formula can increase the level of BMD in lumbar spine.(5)Bone tissue morphology:By obsering HE and Masson, compared with the normal group,the number of trabecular bone in model group significantly reduced,the connection in the trabecular bone was incomplete, and diameter of trabecular bone becomed thin, even fractured. In test group, the number of trabecular bone increased, and was similar to the normal group, the connection in trabecular bone was tidy, the space of trabecular bone was decreased.(6) The parameters of the biomechanical experiments:The maximum load of model group significantly lower than normal group(p<0.01) and the maximum load of text group more higher than model group (p<0.05) in the three-point bending test of femur. Femoral compression test, the maximum load of text group more higher than model group (p<0.05) and the maximum load of model group significantly lower than normal group(p<0.01), the displacement of model group significantly lower than normal group(p<0.01), the displacement of text group higher than model group,but there were no significant differences in each. Lumbar compression test, the maximum load of text group more higher than model group (p<0.05), the displacement of text group significantly higher than model group(p<0.01).(7) Ultrasound can be a direct response to the mechanical properties of bone and trabecular bone microstructure changes.It can be further applied in the basic experiment.Conclusions(1) The pathogenesis of primary osteoporosis is deficiency of kidney and blood stasis.(2)the weight of rats significantly increased, diameter of trabecular bone becomed thin,BMD decreased after removing ovarian.It can reflect the model of postmenopausal osteoporosis.(3)Huo xue bu shen formula can improved trabecular structure,increased BMD and improved the biomechanical properties. Its mechanism was complex, it need further study in the future.(4) Ultrasound can be a direct response to the mechanical properties of bone and trabecular bone microstructure changes.It can be further applied in the basic experiment.
