Objective:In order to explore the effectiveness to reduces the incidence and the mortality of stroke in community population,a community-based intervention program of stroke was conducted in large scale population through application of whole population strategy and high risk population strategy. Methods:From 1996 to 2006,A community-based preventive program of stroke was conducted in Xietu Community with a population size of about 70,000 people. Main strategies included:①To promotes the schools to take the community inhabitant health as the foundation, and develop health education and the health promotion activities.②To screen and manage hypertensives and the diabetic aged 35 years old above in the community(give medication and non-medication).③To train the community health station workers, and the personnel with the knowledge and skills of cerebrovascular disease prevention and controls in the community.④To train each personnel of disease prevention and control network member's in the neighborhoods with knowledge and skills of reporting cases of cerebrovascular disease.⑤To carry on the intensive health education and health promotion to unhealthy habits and customs. The community-based monitoring of cerebrovascular disease morbidity and death were simultaneously carried on,and quality control of cerebrovascular disease case report was performed among the reporting personnel and in the hospitals..All firsthand information was inputted the EpiData database,to analyzes to two groups of rate comparisons uses the possion x~2 test. Results:In the passed 10 years of the intervention, intervention community in which the total incidence and the mortality of stroke continuously declined. In 2006, stroke incidence of the intervention community was 105.7/100000,mortality was 26.4/100000.Incidence rate has declined by 38.9%compare with that 1996.Mortality has declined by 52.8%.The total incidence and the mortality of stroke, however, continuously increased in control community. In 2006,stroke incidence of the control community was 462.5/100000,mortality was 72.5/100000.The incidence has rose by 370.3%compared with that 1996.Mortality has rose by 88.8%.Conclusion:The present results showed that the intervention program can significantly reduce the incidence and mortality of stroke, and indicated that it is suitable to conduct community-based comprehensive preventive program of stroke. Comprehensive health education and health promotion,as well as hypertension management were the intervention key points. But the effectiveness was not very good for the young people,so it was needed to strengthen the study about intervention measure suitable for young who were employed.
