Background neurogenic bladder is the urethra and bladder dysfunction caused by the damage of the central or peripheral nervous system, which regulation and control voiding function. According to the part of injury, neurogenic bladder is divided into upper motor neuron pattern and lower motoneurons pattern and mixed pattern, upper motor neuron pattern is also called detrusor hyperreflexia, lower motoneurons pattern is also called detrusor areflexia. Spinal cord injury is the frequently cause of neurogenic bladder.At present the treatment mainly including oral medication, bladder drug perfusion, surgical and sacral nerve stimulation, but there are side- effects and complications of surgery drug therapy and invasive urologic surgery. At the same time large clinical study showed that acupuncture therapy has a good effect in treat detrusor hyperreflexia, and with the feature of low-cost, simple operation, less pain, fewer side effects, has a broad range of applications. But the mechanism of acupuncture therapy t in detrusor hyperreflexia is unclear.There are significant differences of micturition reflex between the normal animals and the animals with spinal cord injury. In normal individual, the excitement of bladder afferent is mainly by the myelin afferent nerve conduction (Aδfiber), Aδfiber transmit the feeling of excitement to the brain stem through the spino-thalamic tract, then the feeling of excitement is transmitted to the brain of periaqueductal gray matter, therefore, the condition of bladder filling is transmitted to micturition center in bridge of varolius. However, after spinal cord injury, the capsaicin-sensitive amyelination C nerve fibers cause micturition reflex increased. Thus, the overaction of C afferent nerve fibers is the main pathological mechanism of neurogenic detrusor hyperreflexia. VR1 usually expressed in the C nerve fibers, the increase of VR1 expression can be used as the index of C fibers activities, through VR1 expression to reflect the information of the C fibers activities.Electro-acupuncture on "Ciliao Point" treat detrusor hyperreflexia may through inhibit the C fibers over activities in bladder, the reduce of VR1 expression can reflect the C fibers by inhibition activities.Objective This study initial probe the mechanism of electro- acupuncture treat detrusor hyperreflexia by determine the VR1 expression on brain, medulla oblongata and spinal cord by the detrusor hyperreflexia model of spinal cord transected rat.Materials and Methods the 9 of healthy adult female SD rats were randomly divided into three groups, each group with three, including the treatment group: transect the spinal cord and postoperative care, after urinary incontinence (after two weeks) started acupuncture therapy. Control group: do not for acupuncture therapy. Normal group: no any intervention. Model Building: conventional anesthesia, cut off the rats T10 vertebral plate, complete transect the spinal cord, to set up the model of detrusor hyperreflexia. Before the rats urination can be independently prior to artificial abdominal massage auxiliary urination, when all rats in the treatment group urinary incontinence, acupuncture on "Ciliao Point", then fixing electro-acupuncture device, once a day, every 2h, the course is two weeks in all. After treatment, cardiac perfusion fixation, respectively, from the brain, and the medulla oblongata, L4-L6 spinal cord segments, embedded in OCT, cut sheet by freezing microtome, immunohistochemical staining of VR1, measure the VR1 expression of intensity by image analysis system.Results The VR1 expression in the control group rat spinal cord and medulla oblongata L4-L6 all segments were significantly increased than the normal group', the difference has statistically significant. And the VR1 expression in the treatment group rat brain, medulla oblongata L4-L6 all segments were significantly decreased than the control group', the difference has statistically significant. The VR1 expression in spinal cord between treatment group and control group has no significant difference.Conclusion These results suggest that the electro-acupuncture therapy reduced VR1 expression that indicate the C fibers activities inhibited, suggested that electro-acupuncture on "Ciliao Point" treat detrusor hyperreflexia through inhibit the C fibers over activities in bladder.
