Objective: To observe the application of serum tumor markers in diagnosis and prognosis of col-orectal carcinoma and try to use appropriate combination to increase the sensitivity, specificity and validity in tumor diagnosis. By detecting the levels of the blood serum tumor markers between the preoperative and postoperative subjects , metastasis and recrudescence are expected to indicated in early stages. The role of chemotherapy and the prognosis were predicted by examining serum tumor markers in the earlier stage of col-orectal carcinoma.Method:Five kinds of serum tumor markers including AFP, CEA, CA19-9, CA72-4, and CA242 were detected in 140 patients with colorectal carcinoma and in 33 patients with benign colorectal diseases from 2001 to 2005. And the patients with colorectal cancer were followed up for 2 years. The data were analyzed by SPSS13.0Results:①The levels of 4 kinds of tumor markers were higher in cases of phases ofⅣandⅢthan in those ofⅠandⅡ; and higher in distant metastasis cases and as cites cases (P<0.05). The level of 4 kinds of tumor markers were higher in cases of low-differentiated adenocarcinoma and undifferentiated carcinoma than in cases of high-differentiated and middle-differentiated adenocarcinoma and mucus-adenocarcinoma (P<0.05).②The levels of 4 kinds of tumor markers were higher in cases of the preoperation than in those of postoperation (P<0.01). The simultaneously combined assay raised sensitivity, and the serial combined assay raised specificity.③The levels of 4 kinds of tumor markers were higher in cases of metastasis and recurrence in 2 years of followed operation. Through chemotherapy,the levels of the blood serum tumor markers descend (P<0.05) ,and the time to progress were 5.6 monthes.④The level of AFP had no marketable changes in cases of metastasis and recurrence including liver metastasis .Conclusion: The levels of CEA, CA19-9, CA724 and CA242 are of value in diagnosis, and can also be used to evaluate treatment effect and to monitor recurrence of metastasis in colorectal carcinoma. The combined assay can raise sensitivity, specificity and validity in diagnosis. The judgment value to the metastasis and recrudescence can be indicated by monitoring the levels of tumor markers of the 4 kinds of tumor markers which were higher in cases of the pre-operation and in cases of metastasis and recurrence, so the markers have a early warning effect. The levels of the high tumor markers descend through chemotherapy, but have no effect on normal level levels of the markers and prolong survival time in middle and late stage col-orectal carcinoma patients. By monitoring the tumor markers can be enhance the curative efficacy, evaluating survival time and status of tumor The level of AFP had no marketable changes in cases of both metastasis and recurrence include live metastasis.
