| Purpose:Abundance of clinical evidence proof that the concurrent chemoradiotherapy is better than conventional fraction radiotherapy in esophageal carcinoma and the concurrent chemoradiotherapy is becoming one of the standard treatment modality of upper and middle segments esophageal carcinoma.3-Dimensional conformal Radiation Therapy(3D-CRT) can protect the normal tissues and organs around esophageal carcinoma lesions as much as possible when killing the cancer cells.Much data proves that 3D-CRT can bring much better effect than the conventional fraction radiotherapy in esophageal cancer.Furthermore,the main modality of concurrent chemoradiotherapy is the conventional fraction radiotherapy plus the concurrent chemotherapy at recently.And 3D-CRT plus the concurrent chemotherapy should be better than the conventional fraction radiotherapy does theoretically.However,limited data has bee offered to prove that.This research aims to analysis the difference of short term effect and acute side effect among conventional fraction radiotherapy,conventional concurrent chemoradiotherapy and 3D-CRT concurrent chemoradiotherapy in upper and middle segment esophageal carcinoma so as to prove that 3D-CRT concurrent chemoradiotherapy has much better cure effect than the other tow,and its side effect is endurable.Methods:To collect the information of 60 esophageal cancer patients checked in the second hospital of Xi'an Jiaotong Universtiy during sept.2004 to sept.2006.All the case have complete pre-treatment evaluation,therapeutic effect assessment and follow-up,as well as the archived barium meal radiography and chest CT.The patients are sorted to conventional fraction radiotherapy group,conventional concurrent chemoradiotherapy group and 3D-CRT concurrent chemoradiotherapy group,which have 20 patients in.The barium meal radiography is used as the methods of therapeutic effect assessment and the acute side effect is observed, which include:gastrointestinal reaction,hematological toxicity,radiation esophagitis and radiation pneumonitis.Results:1.The conventional fraction radiotherapy group have CR rate of 20.0%,PR 50.0%,and total RR(CR+PR) of 70.0%and have the least acute side effect.2.The conventional concurrent chemoradiotherapy group have CR rate of 35.0%,PR 45.0%,and total RR(CR+PR) of 80.0%but have the most severe acute side effect.3.The 3D-CRT concurrent chemoradiotherapy group have CR rate of 45.0%,PR 45.0%, and total RR(CR+PR) of 90.0%and have the middle level of acute side effect.Conclusions:1.The 3D-CRT concurrent chemoradiotherapy has better therapeutic effect than conventional fraction radiotherapy and conventional concurrent chemoradiotherapy,and the result has statistical difference(P<0.05).2.The acute side effect of 3D-CRT concurrent chemoradiotherapy is not severe than the conventional fraction radiotherapy and apparently less than the conventional concurrent chemoradiotherapy,which can be tolerable to patients.3.The 3D-CRT concurrent chemoradiotherapy is a effective new modality of treatment to esophageal cancer and worth to be tried in esophageal cancer patients. |
