By Observation And Treatment Of Radial Artery Spasm In Radial Pci

Objective: To observe the effect of nitroglycerin, Verapamil and diazepam preventing radial artery spasm which undergoing intervention through radial artery.Methods: 1200 patients were selected randomly in Bei jing An Zhen hospital, from 10/2005 to 11/2006. They were randomized into 4 groups (A, B, C, and D). Group A were given nitroglycerin 200μg and Verapamil 250μg through sheath. Group B were given nitroglycerin 200μg. Group C were given Verapamil 250μg. Group D were given normal sodium as control. Each group was divided into two subgroups (N1 and N2, N refers to A, B, C, and D). Group N1 was given diazepam 5mg i.m. half an hour before operation, and Group N2 was not given diazepam. The radial artery spasm was observed in different groups, and in subgroups. The recovery time in treated group and control was observed when spasm occurred.Results: 1.The fact involved patient were 1167, median age 59.5+/-13.7years, 591males, 576 females. 2.The ratio of radial artery spasm A1( 3.4%), A2(4.1%), B1(4.2%), B2(5.4%), C1(12.4%), C2(14.8%), D1(23.6%), D2(33.8%) , there was no difference between subgroups (P>0.05). 3. The ratio of radial artery spasm between subgroups in different groups were A1< B1< C10.05), and there was no difference, and A1 VS C1(P<0.05),A1 VS D1(P<0.05),B1 VS C1(P<0.05), B1 VS D1(P<0.05),C1 VS D1(P<0.05). 4. The recovery rate between treatment and control was different(P<0.05).Conclusions:1. Nitroglycerin,verapamil,and nitroglycerin add verapamil can reduce the ratio of radial spasm and prompt recovery of spasm.2. The ability of nitroglycerin preventing spasm was better than verapamil.3. The effectiveness of preventing spasm nitroglycerin add verapamil was better than nitroglycerin alone, but there`s no statistic difference, and nitroglycerin add verapamil was better than verapamil, there was statistic difference.4. The patients received diazepam had lower ratio of radial artery spasm than control in treated groups with nitroglycerin,verapamil,and nitroglycerin add verapamil, but there was no significant difference.5. The ratio of spasm was lower than control normal sodium subgroup in the control subgroup that given diazepam before operation. There were difference for female patients in group D who received diazepam than not, and there was no significant difference for male.6. The recovery time was shorter in treated groups with nitroglycerin,verapamil,and nitroglycerin add verapamil, when given nitroglycerin add verapamil once more after spasm. So we can get a conclusion that use some drugs to prevent radial artery spasm is important and necessary.
