All that make medical expense hold an invalid control and the public medical treatment lack a necessary guarantee system.This article combines advanced management theories of foreign medical field, looks back our special urban reform processes and makes use of Antinomy analysis methods to analyse difficult problems urban medical system's reform faces to. It discusses the government role in the medical field and how the government and the market can develop respective advantages, then considers that medical treatment organizations need to head for the market transition filled with management and the government and the market need to cooperate with each other.System's reform of the urban medical treatment had better to commence from both side of the medical service, supplyer and demand side, improve the construction of basic medical service system, increase government input to medical field, strengthen the charging ability, divide responsibility scopes of the government and the market in medical field correctly and improve basic medical insurance system and the methods of payment of medical expenses.A new round of medical reform doesn't require to go back to the old system that the government pays for everything. In the context of the market economy, depending on the government completely would make the government out of order. Feasible ways are to partly open the medical market to the private, taking advantages of market operation, actively promoting the service competition, reasonable pricing, to divide responsibility scopes of the government and market, to make the two parts complete each other and cooperate with each other, using economy, politics, social management and other means to adjust and perfect the medical system continuously. That would betterly meet various medical demands of all classes in the society, carry out the effective management of medical treatment field and finally provide everyone to possess a basic medical service.
