This is the literature review article which focus on the understanding that western medicine to the handicapped microcirculation of coronary artery and the treatment that traditional Chinese medicine to its causes and pathogenesis.Formerly, the research on coronary artery always focuses on the epicardium. The research on microcirculation coronary artery does not arouse much attention. In clinical research, we find the following phenomenon: amount the patients of acute coronary artery syndrome, after long time obliterative coronary artery, though percutaneous coronary intervention (PCI) can rebuild the blood flow in the vessel under the epicardium, the phenomenon of"no-reflow"will show up. That means the cardiac muscle have no level reperfusion. The patients who suffering non-blockage CAD or cardiomyopathy will have the same clinical situation of heart muscle ischemia. High blood pressure combines with myocardial hypertrophy can make the patient easily get heart muscle ischemia symptom. Researching on these phenomenons, people pay more and more attention to microcirculation of coronary artery. The latest research certificate that the decrease of coronary artery flow reserve, the"no-reflow"phenomenon after the CAD revascularization and the syndrome X that the high blood pressure patients have relative with the affection of coronary artery microcirculation.This article concludes and analyses the meaning of moral microcirculation of coronary artery: structure, function and the evaluation method of microcirculation of coronary artery dysfunction. Finally, we summarize four classification of coronary microcirculation dysfunction: 1. The dysfunction occurs in the absence of CAD and myocardial diseases. 2. Dysfunction in the presence of myocardial diseases. 3. Dysfunction in the presence of obstructive epicardial CAD. 4. Iatrogenic dysfunction.Currently, there are few efficient methods and medicines in western medicine to cure handicapped coronary artery microcirculation. We should make use of traditional Chinese medicine and exert its advantage. So we can find efficient cure methods to improve the dys-prognosis of the patients of handicapped coronary artery microcirculation.The traditional Chinese medicine believes that microcirculation is similar to collaterals and subcollaterals in traditional Chinese medicine. The heart collaterals are similar with the coronary artery microcirculation. By long time clinical practice and observe, we find out that: cure the patients of CAD by interventional therapy, a big percentage of them have the complaint of early stage cardia insufficiency. The phenomenons are: the complexion is peak and pine; the symptom is cardiac insufficiency after tired (debilitation, the cun pulse is sunken and small on left hand) and chest complaint and heart blood stasis (body of tongue gloom or light gloom). These symptoms always show in the patients who suffer high blood pressure combines with myocardial hypertrophy or cardiomyopathy. All these diseases above belongs to the symptoms of deficiency of vital energy, blood stasis and damage heart collaterals. So the symptoms of deficiency of vital energy, blood stasis and damage heart collaterals are the familiar pattern of syndrome in dysfunction of the coronary artery microcirculation in traditional Chinese medicine. The investigations prove that use the Chinese crude drug to invigorate vital energy and promoting blood flow can improve the microcirculation. The efficiency of Chinese crude drug is obvious in clinical treatment. Traditional Chinese medicine and Chinese crude drug have their own advantages to cure the above diseases.All in all, abnormalities in the function and structure of the coronary microcirculation occur in many clinical conditions, especially after interventional therapy of coronary artery. Though we pay much more attention to the functional disturbance of coronary artery microcirculation, good result has been made to the research of single medicine and complex prescription. Step up the research on the dysfunction of the coronary microcirculation by combining the differential diagnosis of diseases and the differentiation of symptoms and signs; finding efficient cure methods and medicines; seek for the best time of using Chinese crude drug; obviating the disease of the coronary microcirculation dysfunction; preventing it from dysfunction to structural disturbance. So that we can exert the advantage of"following the prognosis of a disease"of the Chinese crude drug and exert more affectation of combination of TCM with Western medicine to the micro-affection management. That will step up the cure method of cardiovascular disease by using the combination of TCM with western medicine.
