The complications,contraindication as well as clinical outcomes due to various alternatives such as anterior den screw fixation,anterior transoralpharyngeal atlantoaxial reduction plate,posterior C1-C2 transarticular screw fixation,posterior wiring techniques,posterior pedicle screw instrument in atlas,posterior apofix techniques,anterior percutaneous screw fixation are reviewed comparatively to supply the guidance to the treatment.Anterior den screw fixation can give compression to the den fracture with high fracture union low complication, which is the the best alternative in the treatment of den fracture. Only in the contraindication of anterior den screw fixation, we use posterior atlantoaxial fusion,try our best to achieve fracture union and reserve activity of cervical spine.
