.th1/th2 Cytokines Drift Clinical Significance In Gastric Cancer Patients

Objective:Through detecting the levels of Th1 and Th2 type cytokines of peripheral blood CD⁴⁺T lymphocytes in the patients with gastric cancer,to observe the shift of Th1/Th2 in the patients with gastric cancer,the features of Th1,Th2 cytokines in patients with gastric cancer of different patho cell differentiation degrees,the relationship between Th1,Th2 cytokines and the metastasis of gastric cancer,the change of Th1,Th2 cytokines before and after chemotherapy,and the relationship between the change and the therapeutic effect.Approach the relationship between Th1,Th2 cytokines and prognosis.Method:Collecting the peripheral blood examples of research objects,the levels of IFN-γ,TNF-α,IL-2,IL-4,IL-6 and IL-10 were detected by flow cytometrys of Th1 Th2 cytokine markered.The data were analyzed by SPSS11.0.Result:1.The levels of IFN-γ,IL-2 of peripheral blood CD₄⁺ T lymphocytes in gastric cancer patients were obviously lower than those in normal control(p＜0.01).Th1/Th2(IFN-γ/IL-4)was lower too(p＜0.01).But the levels of TNF-α,IL-4,IL-6,IL-10 were significantly higher than those in normal control(p＜0.01).2.The levels of IFN-γ,IL-2 of gastric cancer patients steped down gradually accompany with the degrade of cell differentiation degrees.But the levels of IL-4,IL-6 steped up gradually Th1/Th2(IFN-γ/IL-4)steped down gradually too,there were significant differences among the poorly differentiated,the moderately differentiated and the well differentiated adenocarainoma(p＜0.05).3.The levels of IFN-γ,IL-2 of gastric cancer patients steped down gradually accompany with the increase of the TNM stages.The levels of TNF-α,IL-4,IL-6,IL-10 steped up gradually.Th1/Th2(IFN-γ/IL-4)steped down gradually too,there were significant differences between stageⅡandⅢ,stageⅡandⅣ(p＜0.01).4.The levels of IFN-γ,IL-2 of peripheral blood CD₄⁺ T lymphocytes in gastric cancer patients steped down gradually accompany with the aggravate of metastasis extent.But the levels of TNF-α,IL-4,IL-6,IL-10 steped up gradually.The level of Th1/Th2( IFN-γ/IL-4)steped down gradually too.5.There were no differences of the levels of TNF-α,IL-2,IL-4,IL-6 and Th1/Th2 (IFN-γ/IL-4)before and after chemotherapy(P＞0.05).The level of IFN-γafter chemotherapy is higher than that before chemotherapy(p＜0.05).The level of IL-10 after chemotherapy is lower than that before chemotherapy(p＜0.05).The level of IFN-γ,TNF-α,Th1/Th2(IFN-γ/IL-4)in those whose therapeutic effect are PR after chemotherapy is higher than that before chemotherapy(p＜0.05) Conclusion:1.It states Th2 dominance in peripheral blood CD₄⁺ T lymphocytes of gastric cancer patient,Th1/Th2 disequilibrium which leads to immune function hypofunction.It is one of the reasons of tumor recur or tumor metastasis possibilily.2.The levels of Th1 type cytokines IFN-γ,IL-2 step down gradually,Th2 type cytokines IL-4,IL-6 step up gradually accompany with the degrade of cell differentiate degrees.The shift of Th1/Th2(IFN-γ/IL-4)aggravate accompany with the degrade of cell differentiate degrees too.3.The levels of Th1 type cytokines IFN-γ,IL-2 step down,Th2 type cytokines IL-4,IL-6 step up gradually accompany with the increase of the TNM stages.The shift of Th1/Th2(IFN-γ/IL-4)aggravate accompany with the increase of the TNM stages.4.The levels of Th1 type cytokines IFN-γ,IL-2 step down gradually,Th2 type cytokines IL-4,IL-6 step up gradually accompany with the aggravate of metastasis extent. The shift of Th1/Th2(IFN-γ/IL-4)aggravate accompany with the aggravate of metastasis extent.5.Effective chemotherapy can lessen tumor load,lessen the restrain of tumor to immune function,relieve the shift of Th1/Th2.But the effective power of gastric cancer chemotherapy is fairly low,and the digestive tract function of gastric cancer patients is poor,therefore to improve immune function and reestablish the defense system is very significant therapeutic measure to gastric cancer patients especially to chemotherapy patients.At the same time monitoring the cytokines of CD₄⁺ T lymphocytes have positive effect to estimate prognosis and instruction clinic chemotherapy.6.The significance of detecting Th1,Th2 types cytokines is not the diagnosis of diseas,but the observation and analysis immune state and the development of diseas, adjust the balance of Th1/Th2,work out propotional treatment plan.
