| Objective 1. To study the difference of Adiponectin and Leptin level compared newly diagnosed type 2 diabetic patients with normal glucose tolerance. 2. To investigate the changes of Rosiglitazone(ROS) on Adiponectin and Leptin level in patients with type 2 diabetes before and after treatment. To discuss the relation between serum Adiponectin and Leptin, and their relations of obesity and insulin resistance.Methods 1. Forty-six cases of type 2 diabetic patients were recruited according to WHO criterion (1999), and forty-three cases with normal glucose tolerance were simultaneously recruited to measure the height, weight, waistline, hipline. Then the BMI and WHR of those two groups were calculated. Blood samples were fasting collected in the morning to be used in measuring plasma glucose and lipids. Use the abstracted serum, which be reserved in the refrigerator at minus 70 degrees Celsius, to investigate the fasting insulin, proinsulin(PI), adiponectin, and leptin concentration. To observe the difference between the indices of those groups, and to analyse the relation between serum adiponectin and leptin, and their relation with obese and insulin sensitivity. 2. Treat 38 cases of these patients of ROS 4mg each day. Repeat these testing indices after 4-week treatment. And compare the changes in insulin sensitivity, serum adiponectin and leptin level. Discuss the relationship of adiponectin and leptin, and obese and insulin sensitivity. 3. Insulin sensitivity was evaluated by using Homeostasis Model Assessment(HOMA) formula.. HOMA-IR=FINS×FPG/22.5.Results Levels of fasting serum adiponectin of Diabetes' group were significantly decreased(P < 0.05) , the values of adiponectin, HOMA-IR ,BMI and WHR increased significantly (P< 0.01) after 4 weeks' ROS treatment. The levels of serum leptin before and after treatmenthave no significant difference. the level of serum adiponectin in patients with type 2 diabeteshas significant contrary correlation with fast plasma glucose, serum insulin and HOMA-IR,notablely, the level of serum leptin in patients with type 2 diabetes has significant correlationwith BMI,fast serum insulin and HOMA-IR. Adiponectin has no significant correlation withleptin.Conclusions The level of serum adiponecyin has significant effect in patients with type 2 diabetes, but leptin has little effect. Leptin has the correlation fat. Our results show that adiponectin and leptin may play important role on the mechanism of type 2 diabetes and human insulin resistance. |
