Clinical study of influence factor for curative effect of sciatica by psoascompartment block under thrmography .CT guide.Postgraduate :MA Song_he Teacher: professor Yan Xiang_MoObjective:To study of influence factor for curative effect of sciatica by posas compartment block using thermography ,CT guide fixe puncture point ,depth of puncture needle ,inject anethetic drugs, go further into interrelated factor for treatment curative sciatica by psoas compartment block.Methods: thirtytwo sciatica by disc hernia of L₆^S₁ patients landomly divided into two groups ,group 1, 15cases ,no inject air into psoas compartment (NO AIR); group 2 ,17 cases, inject air into psoas compartment (AIR). All patients of the two groups, 20min of after and befor inject anesthetic drugs ,end of treatment course ,measure of leges temperature by thermography . In order to exactly measure depth of puncture needle and rang of diffusion in the NO AIR and AIR ,the operate were observed with VAS at before and after treatment ,determin the effect of pain treatment.Results :(1)depth of puncture needle were no significant difference between males and females patients .(2)diffusion range of anesthetic drugs ,group 2(AIR) was signifficantly wide than group 1(NO AIR) ,P< 0.01. (3) legs temperature were as follows: after 20 min of injectanesthetic drugs and after end of treatment, significantly high than before inject(P<0.01) , but no significant difference between two groups (P> 0.01). (4)after end of treatment ,pain score with VAS were significantly descend than before treatment (P<0.05), but no significant difference no interrelated with diffusoin range of injected drugs .Conclusion :(l)fixed puncture point and needle depth is very important when treatment of sciatica with psoas compartment block ,it must be correct fixed puncture point,according to guide with thermography ,CT in this study .the depth in males were 6.2 +0.72cm, females were 5.8 ± 0.62cm ,no significant difference (P<0.01),but the effect no significant difference between two groups.(3)It is very safety and efficacy method that psoas compartment block for sciatica .(4)CT guide were very useful for fixed puncture point exactly .(5)thermography is an strong objective basis for judge curative effect of sciatica by psoas compartment block.
